Abstract

Introduction: Elevation of inflammation markers and changes in neutrophil/lymphocyte distribution are evaluated in patients with acute ischemic stroke. Recently, the neutrophil/lymphocyte ratio has been showed as a predictor of short-term functional outcome and mor- tality in patients with ischemic cerebral damage. The aim of the study was to investigate whether neutrophil/lymphocyte ratio is related to silent cerebral infarction in patients underwent diagnostic coronary angiography and/or percutaneous coronary intervention. Material and Method: A total of 290 consecutive patients who underwent diagnostic or interventional coronary procedures were eval- uated for silent cerebral infarction. Neuron specific enolase levels were studied before and 12 hour after the procedure. Elevation of greater than 12 ng/mL was considered as silent cerebral infarction. White blood cell counts and peripheral differential counts were measured on admission. Results: Patients were mainly male (61%) with a mean age of 62 years. Eighty of 290 study patients (28%) had silent cerebral infarct after the procedure. When patients were divided into two groups according to silent cerebral infarction, patients with silent cerebral in- farction were more likely to have high neutrophil/lymphocyte ratio (4.5 in patients with SCI and 3.3 in patients without silent cerebral infarction, p<0.001). Patients with SCI were more likely to have male sexuality, hyperlipidemia, history of smoking and prior myocardial infarction. All patients were classified into tertile groups based on neutrophil/lymphocyte ratio after coronary procedure. Patients in higher tertiles of neutrophil/lymphocyte ratio were likely to have higher silent cerebral infarction prevalence. Conclusions: The present study suggests that neutrophil/lymphocyte ratio is higher in patients with silent cerebral infarction after di- agnostic coronry angiography and/or percutaneous coronary intervention.
