Abstract

Objective: In many observational studies; atrial fibrillation is more common in chronic obstructive pulmonary disease (COPD) than in the general population. Although many mechanisms have been proposed to explain this association, the relationship between right atrial dilation and atrial fibrillation has not been investigated. In this study, we aimed to investigate the relationship between right atrial di- latation and atrial fibrillation in chronic obstructive pulmonary disease patients. Material and Method: 61 chronic obstructive pulmonary disease patients who were treated in-patient between June 2014 and June 2015 were included in this study. Of the patients, 36 had sinus and 25 had atrial fibrillation rhythm. Computerized thorax tomography was used to measure the atrial size in order to evaluate right atrial dilatation. Tricuspid and mitral annulus diameters; tricuspid annulus diameter/mitral annulus diameter ratio were calculated using computerized tomography in both groups. Patients with organic tricuspid valve disease, severe aortic and mitral valve disease, valve prosthesis, and hemodynamically unstable acute/chronic pulmonary embolism were excluded from the study. Results: Patients in the atrial fibrillation group were older, hypertensive, and had more heart failure. Systolic pulmonary artery pressure measured by echocardiography was significantly higher in the atrial fibrillation group (p <0.001). It may show right atrial dilatation; tri-cuspid annular diameter and tricuspid annular diameter/mitral annulus diameter ratio were significantly higher in the atrial fibrillation group (3.16 cm ± 0.60; 4.62 cm ± 0.8 p <0.001/0.85 ± 0.12; 1.26 ± 0.15 p <0.001). Tricuspid annular diameter greater than 3.4 cm was associated with a twofold increase in atrial fibrillation (OR 2.0 95% CI 1.51-2.63; p = 0.002). In addition, sensitivity and specificity of pre- dicting atrial fibrillation in chronic obstructive pulmonary disease patients with tricuspid anulus diameter of 3.4 cm and over were found to be 92% and 67%, respectively. Conclusions: In this study, we found a relationship between atrial fibrillation and right atrial dilatation in patients with chronic obstruc- tive pulmonary disease, which was not noted in previous studies. Unlike other diseases, right atrial dilatation and remodeling may play a role in the pathogenesis of atrial fibrillation in patients with chronic obstructive pulmonary disease, as well as pulmonary venous trig- gering. In addition, in the diagnosis and follow-up of chronic obstructive pulmonary disease, the right atrial dilatation in chest tomog- raphy can determine and treat the patient group with high risk of atrial fibrillation.

Hastaların 36'sı sinüs, 25'i atriyal fibrilasyonun ritmindeydi.
