Abstract

AIM: The clinical role of the coronary collateral circulation has been a subject of great interest and controversy for many years. The factors on the development of coronary collaterals and the functional significance of collateral circulation remain unclarified. This study was designed firstly to evaluate the circumstances that promote coronary collateral circulation and secondly to determine the functional importance of well developed coronary collateral vessels. METHOD: 121 patients (98 male, 23 female) who have at least %90 stenosis in one of the three major coronary arteries were included in the study. Echocardiography, left ventriculography, and coronary angiography were performed to all patients for diagnosis of coronary artery disease and for evaluation of coronary anatomy and coronary collateral circulation. RESULTS: 40 (33.1%) were detected to have poorly developed collateral vessels whereas in 81 (66.9 %) of the patients well developed vessels were visualized. Univariate and logistic regression analysis showed that total occlusion, critical right coronary artery lesion and smoking had independent value for prediction of adequate collateralization. In addition we found significantly better the hemodynamic parameters in patients with adequate collateralization (AC) that in those with inadequate collateralization (IC). CONCLUSION: These results suggest that total occlusion, critical right coronary artery lesion and smoking is important markers of AC and that the presence of AC may protect left ventricular function.
