Abstract

AIM: It has been reported that increase in QT dispersion (QTD) in acute myocardial infarction (AMI) might be associated with prognosis. This study was carried out to evaluate the role of QTD to predict postinfarction angina and ventricular tachyarrhythmias in AMI. MATERIAL AND METHODS: 194 cases with AMI applied during the first twenty four hours after chest pain were included in this study. QTD was calculated from EKGs which were recorded on admission and during postinfarction angina (QTD=QTmax-QTmin). According to Bazett's formula, corrected QT dispersion (QTcD) was calculated. The cases were classified as ones with (group A, n=53) and without (group B, n=141) postinfarction angina. Group A was divided into 2 subgroups as ones with (group A1, n=14) and without (group A2, n=39) developing ventricular tachyarrhythmias. RESULTS: QTD and QTcD were higher in group A than in group B (p<0.05). QTD and QTcD measured during angina in group A were more increased than the values measured on admission [(56.6±21.3 ms vs 51.3±19.8 ms, p=0.018) and (66.4±24.4 ms vs 60.6±22.8 ms, p=0.026) respectively]. QTD and QTcD were found more increased in group A1 than in group A2 (p<0.05). CONCLUSION: Increased QTD measured on admission in AMI is releated to the increased frequency of postinfarction angina. QTD has been more increased during postinfarction angina. That QTD had higher values measured during postinfarction angina than the values obtained on admission might be helpful to predict the increased risk of ventricular tachyarrhythmias.
