Abstract

Mediastinal thyroid enlargement is one of the rare cause of the tracheal obstruction and central venous return insufficiency. In most case, the central venous insufficiency is secondary to compression of the brachiocephalic vein. Acute respiratory failure and unilateral signs of impaired venous return from the head-neck, upper trunk and upper limb developed after central venous catheter implantation in a 64 year- old male patient. The cause was found to be compression of the trachea and brachiocephalic vein by enlarged thyroid due to bleeding within nodule secondary to injury of the mediastinal multinodular goiter. The symptoms and clinic signs improved by spontaneous reabsorbtion of the bleeding.
