Abstract

Aim: Revascularisation of patients with low ejection fraction has beneficial effects. It prevents progressive left ventricular dilatation, sudden cardiac death, reduction of functional capacity. Coronary artery bypass grafting (CABG) improves long-term survival while relieving angina with a better functional capacity. Material and Method: Thirty seven consecutive patients with low EF (≤30%) who underwent CABG in our department between February 2010 and April 2012 were included in the study Mean age of the patients was 62.32±10.86 (8 women and 29 men). Eight patients underwent of-pump CABG. Results: Twenty-two patients were diabetic (59.5%), 24 patients had hypertension (64.9%), 14 patients had hyperlipidemia (37,8%), 27 patients had a history of smoking (73%), two patients had chronic obstructive pulmonary disease (%5.4), six patients had peripheral artery disease (%16.2), one patient had a history of cerebrovascular accident (2.7%), one patient had a history of intracardiac defibrillator implantation (2.7%), three patients had percutaneous coronary intervention (8.1%). The left internal mammary artery graft (LIMA) was used in 18 patients (48,6%). Full revascularisation rate was 78.3%. The mean number of grafts used was 2.48±0.83. Ten patient had single CABG, 11 patients had double CABG, 14 patients had triple CABG and 2 patients had 4 CABG. The mean number of graft per patient was 2.2. Mean preoperative EF was 25.86%±4.45 (range 15% to 30%). Mean postoperative EF was 35.57%±9.22. Postoperative EF was significantly higher. Left ventricular mass index was found to be improved significantly postoperatively. Preoperative LVEDD was 56.8±7.19 mm and LVESD was 45.46±8.42 mm. Postoperative LVEDD and LVESD were 54.6±7.31 mm and 41.94±8.36 mm consecutively. Sixth and 12th month outpatient mean LVEDD and LVESD were statistically significantly lower and EF was statistically significantly higher than preoperative values (p<0.05). Sixth and 12th month outpatient echocardiographic parameters did not show any significant difference. The patients did have significantly decreased angina and increased functional capacity after the operation. Sixteen patients were in CCS class I/II and 19 patients were in CCS III/IV preoperatively while all patients were in CCS class I/II postoperatively. Eightteen patiets were in NYHA class I/II and 17 patients were in NYHA class III/IV preoperatively while all patients were in NYHA class I/II postoperatively. Postoperative mean values of CCS angina and NYHA functional capacity classifications were significantly lower than preoperative scores (p<0.05). Postoperative mean value of CCS angina class improvement found as 1.35±0.37 and mean value of NHYA functional capacity improvement was 1.33±0.25 when compared to preopative status. In-hospital mortality was 5.4%. One patient died seven months after hospital discharge and mortality was due to heart failure. Annual mortality was 8.1%. Conclusion: Risk factors should be evaluated for the short and mid term outcomes of patients with low EF undergoing CABG.

Bulgular: Yirmiiki hastada diabetes mellitus (%59,5), 24 hastada hipertansiyon (%64,9) 14 hastada hiperlipidemi (%37,8), 27 hastada sigara öyküsü (%73), 2 hastada kronik obstrüktif akciğer hastalığı (KOAH) (%5,4), 6 hastada periferik arter hastalığı (PAH) (%16,2), 1 hastada serebrovasküler hastalık (SVH) (%2,7), 1 hastada implantable cardioverter defibrillator (ICD) öyküsü (%2,7) ve 3 hastada perkütanöz transluminal koroner anjiyoplasti (PTCA) öyküsü (%8,1) mevcuttu. Ameliyat öncesi CCS I/II angina sayısı 16 ve CCS III/IV angina sayısı 19; iken ameliyat sonrasında tüm olgular CCS I/II angina sınıfında gözlenmiştir. Yine ameliyat öncesi NYHA I/II fonksiyonel klas sayısı 18 ve NYHA III/IV fonksiyonel klas sayısı 17 iken ameliyat sonrasında tüm olgular NYHA I/II sınıfında gözlenmiştir. CCS angina ve NYHA efor kapasitesi sınıflaması ortalama puanları ameliyat öncesindeki ortalama puanlarından istatistiksel olarak anlamlı düşük bulundu (p<0,05). Postoperatif CCS angina sınıfında, preoperatif verilere göre ortalama 1,35±0,37, NYHA efor kapasitesinde ise ortalama 1,33±0,25 düzelme saptadık. Olguların hastane mortalitesi %5,4 idi (2 hasta). Her iki hasta da düşük kardiyak output nedeniyle kaybedildi. Hastaların taburculuk sonrası izlemlerinde 7. ayda 1 hastada kalp yetmezliği nedeniyle mortalite görüldü. Yıllık mortalite ise %8,1 saptandı. Sonuç: Sol ventrikül disfonksiyonlu hastalarda uygulanan yüksek riskli koroner baypas cerrahisi sonrası sol ventrikül fonksiyonlarında ve hastanın kliniğinde kısa ve orta vadedeki düzelmeler olguların risk faktörleri de göz önünde tutularak değerlendirilmelidir.
