Abstract

Objective: Coronary heart disease is the leading cause of death in our country as is in the most countries. Changeable risk factors that cause coronary heart disease are smoking, high blood pressure, diabetes, diet, obesity, sedentary lifestyle, hyperlipidemias. In this study, levels of homocysteine, Lp(a) and Von Willebrand factor in male coronary heart disease patients without unchangeable risk factors were evaluated. Material and Method: Male subjects with similar body mass index and age (50-55), without unchangeable risk factors such as age, sex and familial predisposition were included in the study. Levels of homocysteine, lypoprotein (a) and VonWillebrand factor of 10 coronary heart disease patients without changeable risk factors (Group I), 12 coronary heart disease patients with only type 2 diabetes mellitus (Group II) and 52 healthy individuals (Group III) were compared. Results: Homocysteine levels were 16.5 ± 4.1 μmol/L, 17.4 ± 4.1 μmol/L, and 11.8 ± 2.4 μmol/L in Group I, II and III respectively. Lypoprotein (a) levels were 24.5 ± 6,1 mg/dL, 32.9 ± 3.8 mg/dL , and 20.8 ± 4.6 mg/dL in Group I, II and III respectively. Von Willebrand factor levels were 216.5 ± 12.8 mU/mL, 219.6 ± 13.1 mU/mL and 86.9 ± 19.5 mU/mL in Group I, II and III respectively. All three pa- rameters were found higher in both coronary heart diseasegroups compared to control subjects. In two groups of coronary heart dis- ease, lypoprotein (a) levels were significantly higher in Group II, and other parameters were similar. Conclusion: We suggest homocysteine, lypoprotein (a) and Von Willebrand factor should be included into routin parameters in coronary heart disease detection and it is concluded that lypoprotein (a) is significant in diagnosis and following of coronary heart disease with conventional risk factors.

Bu çalışmada değiştirilemez risk faktörleri içermeyen, erkek diyabetik koroner kalp hastalığındaki homosistein, lipoprotein (a), Von Willebrand faktör düzeylerinin belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmamızda yaş, cinsiyet, ailesel yatkınlık gibi değiştirilemez risk faktörleri içermeyen, vücut kitle indeksleri benzer 50-55 yaş aralığında, erkek bireylerden, koroner kalp hastalığı olup herhangi bir değiştirilebilir risk faktörüne sahip olmayanlar (10 has- ta, Grup I), koroner kalp hastalığı olup sadece tip 2 diyabeti olanlar (12 hasta, Grup II) ve kontrol grubu (52 sağlıklı, Grup III) arasında homosistein, lipoproten (a), Von Willebrand faktör düzeyleri karşılaştırılmıştır. Bulgular: Homosistein düzeyleri, Grup I: 16,5 ± 4,1 μmol/L, Grup II: 17,4 ± 4,1 μmol/L, Grup III: 11.8 ± 2,4 μmol/L, Lp(a) düzeyleri, Grup I: 24,5 ± 6,1 mg/dL, Grup II: 32,9 ± 3,8 mg/dL, Grup III: 20,8 ± 4,6 mg/dL. Von Willebrand faktör düzeyleri, Grup I: 216,5 ± 12,8 mU/mL, Grup II: 219,6 ± 13,1 mU/mL, Grup III:86.9 ± 19.5 mU/mL olarak saptanmıştır. Homosistein, Lp(a), Von Willebrand faktör düzeyleri, ko- roner kalp hastalığı olan her iki grupta kontrol grubuna göre anlamlı olarak yüksek (p<0,05), koroner kalp hastalığı olan gruplar kendi arasında karşılaştırıldığında Lp(a) düzeyinin Grup II’de anlamlı olarak arttığı diğer parametrelerin değişmediği görülmüştür. Sonuç: Koroner kalp hastalığının öngörülmesinde, homosistein, Lp(a), Von Willebrand faktör F’nin diğer lipid parametreleri gibi rutin tet- kikler arasında olması gerektiği, Lp(a)’nın konvansiyonel risk faktörü taşıyan koroner kalp hastalığı tanısında daha önemli olduğu, bu hastaların tanı ve takibinde daha etkili olabileceği düşünülmektedir.
