Abstract

Purpose: To evaluate the signs and symptoms and clinical fetures of idiopathic intracranial hypertension (IIH) at the time of diagnosis. Material and Methods: Thirty patients with IIH who were followed at our neuro-ophthalmology department between 1997 and 2002 were included in this study. Modified Dandy's criteria were used for diagnosis. Patients' charts were reviewed retrospectively with special attention given to the detailed symptoms and signs at the time of diagnosis. Results: Twentyseven women and 3 men with a mean age of 35.6 were diagnosed with IIH. Obesity as defined by body mass index was a prominent factor among women (%59.3). The hallmark symptom was positional headache (%90.0) followed by transient visual obscurations (TVO) and pulsatile tinnitus. Papilledema was present in all patients, being symmetrical in 26 (%86.7) and asymmetrical in 4 (%13.3). Visual field defects were present in %75.8, mostly demonstrating arcuate defects, peripheral constrictions and big blind spots. Conclusion: Detailed history-taking and initial examination are crucial in a patient presenting with either headache or papilledema, because presenting symptoms and signs are highly specific and give many clues for the diagnosis of IIH. Neuro-imaging and detection of cerebro spinal fluid (CSF) pressure however, must not be omitted.
