Abstract

PURPOSE: To evaluate 114 cases of unilateral superior oblique palsy, for etiology, clinical findings and surgical results. MATERIALS AND METHODS: In this retrospective study 114 cases of unilateral superior oblique palsy that were examined in our clinic between June 95 and June 99 were included. Diagnosis was confirmed with Parks 3-step test. The patients who had small angle deviation without singnificant head posture and diplopia were treated surgically. RESULTS: Of the patient, 50.9% were congenital and 49.1% were acquired. Hypertropia (15.6 PD) was associated with esotropia in 24.6% of cases, exotropia in 28.0% of cases. Excyclotropia was detected in 32.5% of the cases with double Maddox rod test. Other associated findings were; amblyopia 14%, head posture 79.8%, paradoxical head posture 3.5%. Fusion was present in 43.0% and stereopsis in 33.3% of cases. Postoperatively 16 cases gained stereopsis and fusion. Thirty-five cases had surgery and 62.9% of them had clinical improvement (partial or total improvement in ocular deviation and head posture). CONCLUSION: Superior oblique palsy is the most common cause of cyclovertical deviations, and can be misdiagnosed due to the ignorance of patients and compensation of the problems within tolerable limits. Acquired cases may have spontaneous improvement, however in congenital cases due to progresion surgery can be needed.

BULGULAR: Olguların %50.9 konjenital, %49.1'i edinseldi.
