Abstract

Objective: To evaluate the clinical outcomes of phakic posterior chamber collamer intraocular lens implantation for the correction of high myopia. Material and Method: The medical records of 15 eyes of 8 patients who underwent phakic posterior chamber collamer intraocular lens implantation surgery were reviewed retrospectively. Preoperative and postoperative last visit values of best corrected visual acuity with Snellen chart, spherical and cylindrical refractive errors, intraocular pressure, corneal endothelial counting, central corneal thickness, central and peripheral anterior chamber depth and iridocorneal angle were evaluated. Surgical predictability, reliability and efficiency were calculated. Result: Mean follow-up time was 9.5±8.3(3-23) months. The mean and minimum and maximum spherical equivalent values at the preoperative and postoperative visits were -11.40±3.09 between -7.00 and -16.00 diopter, -0.50±1.27 between +0.25 and -3.00 diopter, respectively (p<0.001). The mean best corrected visual acuity values before and after surgery were 0.72±0.25 and 0.81±0.26, respectively (p=0.033). The mean intraocular pressure and central corneal thickness value changes after surgery were not significant (p=0.73 and p=0.47, respectively). The mean endothelial cell counts were reduced by 2.5% after surgery (p=0.07). Central anterior chamber depth values measured by Oculyzer II, Lenstar and Orbscan II devices were significantly lower than preoperative values after surgery (p<=0.05 for all values). Peripheral anterior chamber depth and iridocorneal angle values were significantly lower than preoperative values after surgery (p<0.05). The efficiency index was 0.94 and the reliability index was 1.12. The predictability was 66.7% and 80% for the postoperative refraction of 0.5 diopter and 1.0 D, respectively. Conclusion: Phakic posterior chamber collamer intraocular lens implantation is an effective and reliable method in high myopia treatment. Peripheral anterior chamber depth and iridocorneal angle along with central anterior chamber depth should be evaluated before phakic posterior chamber intraocular lens implantation.
