Abstract

Fluid therapy is the most important and integral part of the perioperative care. The physiology and pathophysiology of fluid compartments should be considered when fluid therapy is applied. Continuity of hemodynamic stability together with ensuring the adequate intravascular volume is the only effective treatment modality to maintain adequate perfusion pressure. Excessive fluid loading can also have serious adverse consequences as hypovolemia. Intravascular proteins and exogenous colloidal solutions remain in the vascular compartment when the endothelial glycocalyx membrane is in a healthy state. Endothelial damage due to mechanical stress, exposure to endotoxin, ischemia-reperfusion injury, SIRS, sepsis, hyperglycemia, acute hypervolemia causes endocardial glycocalyx damage and interstitial oedema. Therefore, the continuity of normovolemia is important in macrocirculation. Fluid resuscitation should be done in compliance with the guidelines during the perioperative period, and in the conditions of sepsis and burn where endothelial injury is imminent
