Abstract

Lung surgery involves complex procedures associated with high morbidity and mortality, thereby necessitating a systematic, holistic, and evidence-based approach to nursing care. The substantial impact of surgical trauma on respiratory function, the risk of postoperative complications, and the fact that patients are frequently of advanced age with multiple comorbidities require nursing care to be carefully structured and guided by standardized protocols. In the preoperative period, assessment of parameters such as forced expiratory volume in one second (FEV1), diffusing capacity of the lung for carbon monoxide (DLCO), and body mass index (BMI) plays a critical role in predicting surgical risk and developing individualized care plans. Moreover, pulmonary rehabilitation programs, smoking cessation support, optimization of nutritional status, psychosocial assessment, and structured patient education have been reported to reduce postoperative pulmonary complications and facilitate recovery. During the perioperative period, maintenance of normothermia, adequate oxygenation, evidence-based positioning strategies, pressure injury prevention measures, and infection control interventions contribute to enhanced patient safety. In the postoperative phase, early mobilization, effective breathing exercises, multimodal analgesia, balanced fluid therapy, and the use of digital chest drainage systems support lung re-expansion, reduce complication rates, and shorten hospital length of stay. Additionally, structured discharge education, strengthening of self-care competencies, and tele-nursing follow-up promote continuity of care and decrease hospital readmission rates. Overall, the integration of evidence-based nursing interventions with minimally invasive surgical techniques plays a pivotal role in improving patient outcomes, accelerating recovery, and standardizing the quality of care.

Özet

Akciğer cerrahisi, yüksek morbidite ve mortalite riski taşıyan kompleks girişimleri içermesi nedeniyle sistematik, bütüncül ve kanıt temelli bir hemşirelik bakımını zorunlu kılmaktadır. Cerrahi travmanın solunum fonksiyonları üzerindeki belirgin etkisi, komplikasyon gelişme olasılığı ve hastaların çoğunlukla ileri yaş ve eşlik eden komorbiditelere sahip olması, hemşirelik bakımının yapılandırılmış ve standartlara dayalı biçimde planlanmasını gerekli kılmaktadır. Preoperatif dönemde; zorlu ekspiratuvar volüm (FEV1), karbon monoksit difüzyon kapasitesi (DLCO) ve vücut kitle indeksi (VKİ) gibi parametrelerin değerlendirilmesi, cerrahi riskin öngörülmesi ve bireyselleştirilmiş bakım planının oluşturulmasında kritik rol oynamaktadır. Bunun yanı sıra pulmoner rehabilitasyon programları, sigara bırakma desteği, beslenme durumunun optimize edilmesi, psikososyal değerlendirme ve hasta eğitimi uygulamalarının postoperatif pulmoner komplikasyonları azalttığı ve iyileşme sürecini hızlandırdığı bildirilmektedir. Perioperatif dönemde normoterminin korunması, uygun oksijenizasyonun sürdürülmesi, kanıta dayalı pozisyonlama uygulamaları, basınç yaralanmalarını önleyici girişimler ve enfeksiyon kontrol önlemleri hasta güvenliğini artırmaktadır. Postoperatif süreçte erken mobilizasyon, etkili solunum egzersizleri, multimodal analjezi, dengeli sıvı tedavisi ve dijital drenaj sistemlerinin kullanımı; akciğer ekspansiyonunu destekleyerek komplikasyon oranlarını azaltmakta ve hastanede kalış süresini kısaltmaktadır. Ayrıca yapılandırılmış taburculuk eğitimi, öz bakım becerilerinin güçlendirilmesi ve tele-hemşirelik izlemi, bakımın sürekliliğini destekleyerek yeniden hastaneye yatış oranlarını azaltmaktadır. Sonuç olarak, minimal invaziv cerrahi tekniklerle entegre edilen kanıt temelli hemşirelik müdahaleleri, hasta sonuçlarının iyileştirilmesinde, iyileşmenin hızlandırılmasında ve bakım kalitesinin standardizasyonunda belirleyici bir rol üstlenmektedir.
