Abstract

Aim: Taxanes (paclitaxel, docetaxel) constitute the cornerstone of standard chemotherapy protocols for solid tumors. Hypersensitivity reactions (HSRs) occurring during infusion of these agents may lead to treatment discontinuation or life-threatening clinical presentations. This study aimed to comprehensively evaluate the demographic and clinical characteristics, HSR severity according to the Brown classification, and post-reaction management approaches in patients who developed HSRs to taxane-group drugs. Materials and Methods: Thirty patients who developed HSRs during taxane-group chemotherapy (paclitaxel or docetaxel) infusion were retrospectively evaluated. Demographic data, primary diagnoses, comorbidities, cycle number at the time of reaction, and clinical findings were recorded. Reaction severity was categorized as Grade 1 (mild), Grade 2 (moderate), and Grade 3 (severe) according to the Brown classification. Statistical analyses were performed using SPSS 25.0 software. Results: The mean age of patients was 50.87 ± 10.52 years (range: 32–71), and 73.3% (n=22) were female. The most common primary diagnosis was breast cancer (50%). A total of 80% of reactions occurred within the first two exposure cycles. Cutaneous findings (70%) and respiratory symptoms (56.7%) were the most frequently observed phenotypes. A total of 36.7% (n=11) of patients experienced Brown Grade 3 reactions; 81.8% of these patients were first-time taxane recipients. Intradermal skin test positivity was 12.5%. A desensitization protocol was applied in 30% (n=9) of patients. Conclusion: Taxane HSRs continue to cluster within the first two cycles and present with severe clinical features despite widespread premedication use. The low skin test positivity rate supports the predominance of non-IgE-mediated mechanisms. Post-reaction management should be individualized through a multidisciplinary approach based on the phenotype and severity of the reaction.

Reaksiyonların %80'i (n=24) ilk iki maruziyet küründe gelişti.
