Abstract

Aim: To present the clinical course and therapeutic characteristics of a patient who developed Nocardia abscessus keratitis following organic trauma. Case: A 40 year old male patient was referred to our clinic from the emergency department with complaints of redness, tearing, pain, foreign body sensation and decreased vision in his left eye following a plant-related ocular trauma three weeks earlier. Slit-lamp examination revealed ciliary injection and a 3×3 mm epithelial defect with an underlying stromal opacity in the superior nasal paracentral cornea, accompanied by localized conjunctivalization. Corneal scrapings were cultured on Columbia 5% sheep blood agar, chocolate agar, eosin methylene-blue agar and Sabouraud dextrose agar. Fortified fluconazole, vancomycin and amikacin were initiated. On the 8th day of hospitalization, corneal culture confirmed the definitive diagnosis of Nocardia abscessus. Therapy was subsequently revised according to the culture and antimicrobial susceptibility testing. By the 21st day of treatment, the epithelial defect had resolved and the keratitis healed leaving behind a stromal opacity. Conclusion: Nocardia keratitis is a rare clinical condition that responds very well to treatment when suspected based on typical anamnesis and examination findings and early treatment is initiated. Keywords: bacterial keratitis, Nocardia abscessus, organic trauma
