Abstract

Aim: The impact of obesity on chronic spontaneous urticaria (CSU) remains incompletely understood. This study aimed to evaluate the clinical characteristics, disease activity, disease control, and treatment requirements of obese patients with CSU. Materials and Methods: This retrospective single-center study included adult patients with CSU and a body mass index (BMI) ≥30 kg/m² who were followed at the Adult Allergy and Clinical Immunology Department of Ege University Faculty of Medicine between 2014 and 2025. Patients were stratified according to BMI (30–34.9 and ≥35 kg/m²) and treatment modality (antihistamines alone versus antihistamines plus omalizumab with or without cyclosporine). Disease activity was assessed using the Urticaria Activity Score over 7 days (UAS7), and disease control was evaluated using the Urticaria Control Test (UCT). Conclusion: In obese patients with chronic spontaneous urticaria, an increase in body mass index was not associated with disease activity or disease control. However, NSAID-triggered urticaria was observed more frequently both in patients with a BMI ≥35 kg/m² and in those requiring advanced therapy. These findings suggest that obesity may be associated with increased disease triggerability rather than disease severity, and that NSAID sensitivity may represent a marker of a more treatment-resistant clinical phenotype. Detailed evaluation of triggering factors and an individualized treatment approach may therefore be important in the clinical management of obese patients with CSU. Keywords: Chronic spontaneous urticaria; obesity; Urticaria Activity Score (UAS7); Urticaria Control Test (UCT); NSAID-triggered urticaria.
