Abstract
The aim of this study is to better understand the borderline personality structure of Nancy McWilliams' book Psychoanalytic Diagnosis, a 26-year-old female client who had sexual traumas and inadequate object relations in childhood, and to demonstrate how Cognitive Behavioral Therapy (CBT) techniques can be effectively applied to individuals with this level of structure through a concrete case. The main purpose of Cognitive Behavioral Therapy is to enable the individual to recognize dysfunctional thought patterns and develop more realistic and harmonious thoughts instead. The reason the client applied to therapy was the boundary problems in her romantic relationship that had been going on for 1 year and her fears of abandonment. As a result of clinical observation, early traumatic experiences, a chronic feeling of emptiness resulting from discontinuous object relations, the use of primitive defenses, fluctuations in affect, and disruptions in identity integration were observed. The ABC model, awareness of cognitive distortions, Socratic questioning technique, double standard technique, balance model work, relaxation and breathing exercises, the inner child form, and role playing techniques were used as cognitive interventions during the 9 sessions. The findings after the 9-session treatment showed positive changes such as increased self-awareness of the client, increased ability to regulate emotions, and the decision to end a problematic relationship. As a suggestion, in future studies, it is suggested that clients who do not clearly meet the DSM-5 criteria should be evaluated with a psychoanalytic perspective and different therapy schools should be applied.
