Abstract

Aim This case report aims to present the radiographic follow-up and developmental progression of two unerupted supernumerary premolars observed over a four-year period, emphasizing changes in morphology, position, and clinical relevance. Materials and Methods Four panoramic radiographs obtained between March 2021 and May 2025 were compared to evaluate changes in the morphology, developmental stage, and position of the supernumerary teeth. Tooth formation, root development, and positional alterations relative to adjacent anatomical structures were documented. Results The initial imaging revealed two unerupted supernumerary premolars located in the buccal region of teeth #34 and #35. One supernumerary tooth was positioned more posteriorly and buccally. Both teeth exhibited crown formation at the initial examination. Subsequent follow-up radiographs demonstrated continued development of both supernumerary premolars, with noticeable progression in their formation. Positional relationships remained similar; however, morphological maturation was evident. Conclusion Developing supernumerary premolars may exhibit significant morphological progression over time. Periodic radiographic follow-up is essential to monitor their development, anticipate potential complications, and guide appropriate clinical management.
