Abstract

Purpose: Infants with Down syndrome often experience delays in sensory processing, motor development, and broader domains such as communication and social interaction. These multidimensional developmental challenges emphasize the need for comprehensive early intervention models. The Homeostasis–Enrichment–Plasticity (HEP®) Approach is a novel, family-centered intervention grounded in environmental enrichment and ecological developmental theories. This pilot study aimed to evaluate the effects of in-person HEP sessions on sensory, motor, and global developmental outcomes in infants with Down syndrome. Methods: Eight infants diagnosed with Down syndrome (mean corrected age = 9.62 months, SD = 3.15; 2 males, 6 females) received twelve 45-minute face-to-face HEP® sessions over a 3-month period. Developmental changes were assessed pre- and post-intervention using the Ages and Stages Questionnaire (ASQ), the Peabody Developmental Motor Scales–Second Edition (PDMS-2), and the Test of Sensory Functions in Infants (TSFI). Paired-sample t-tests were conducted to analyze pre–post differences. Results: Statistically significant improvements were observed in ASQ fine motor (p = .021) and personal–social (p = .031) domains, while changes in communication (p = .558), gross motor (p = .125), and problem solving (p = .139) were not statistically significant. Significant improvements were also found in PDMS-2 Total Motor Quotient (p = .048), and in TSFI subdomains: Adaptive Motor Functions (p = .003), Visual–Tactile Integration (p = .004), Reactivity to Vestibular Stimulation (p = .005), and TSFI Total Score (p = .001). Conclusion: Preliminary findings suggest that the HEP® Approach may facilitate developmental gains in infants with Down syndrome, particularly in sensory processing and fine motor abilities. Further large-scale controlled studies are warranted to validate the effectiveness of this model.

Bu pilot çalışma, yüz yüze yürütülen HEP® Yaklaşımının Down Sendromlu bebeklerin gelişimsel düzeyleri üzerindeki etkilerini değerlendirmeyi amaçlamaktadır. Metod: Çalışmaya Down Sendromu tanılı sekiz bebek (ortalama düzeltilmiş yaş = 9,62 ay, SD = 3,15; 2 erkek, 6 kız) dâhil edilmiştir. Katılımcılar, üç ay boyunca her biri 45 dakika süren, toplam on iki HEP® yaklaşımı müdahalesine yüz yüze katılmıştır.
