Abstract

Purpose: Pes equinovarus (PEV) is a congenital orthopedic condition affecting foot posture in newborns. The Ponseti method has been shown in the literature as an effective treatment for PEV. After birth, infants undergo a systematic treatment program including serial casting, surgery, and subsequent brace use. The Dennis Brown brace (DBB) is the traditional orthosis used during the treatment process. However, in recent years, modern devices such as the abduction dorsiflexion mechanism (ADM) have become increasingly common. This study aims to evaluate, from the family’s perspective, the challenges, costs, and effectiveness of ADM and DBB braces used after casting and surgery in children with PEV, and to compare parental satisfaction with these devices. Methods: A total of 28 children under the age of three who were treated for PEV in our clinic using DBB (n=16) or ADM (n=12) were included in the study. Families were contacted by phone and asked about demographic information, device usage process, challenges encountered, compliance, perceived treatment effectiveness, and device cost perception, which were rated using a numerical scale. Device satisfaction was also assessed using the Quebec User Evaluation of Satisfaction with assistive Technology (QUEST). Group differences were analyzed using the Mann–Whitney U test. Results: In group comparisons, the ADM group showed significantly higher results in device compliance (ADM:7.5±2.9; DBB:5.8±2.1; U=143.5, p=0.027), cost perception (ADM:7.4±2.8;DBB:3.1±1.9; U=170.0, p=0.001), treatment effect size (ADM: 4.8±3.3; DBB:1.6±3.2; U=142.0, p=0.024), and QUEST scores (ADM: 4.3±0.6; DBB:3.5±0.6; U=160.0, p=0.003). No significant differences were observed in other variables such as device difficulty, general satisfaction, perceived benefit, family burden, and overall treatment satisfaction. Conclusion: While satisfaction and developmental impact were higher in the ADM group, the DBB group provided a cost advantage. The results suggest that ADM devices are superior in terms of patient compliance and overall satisfaction, whereas DBB devices remain advantageous economically.
