Abstract

OBJECTIVE: Red blood cell distribution width (RDW) is a parameter whose level changes with oxidative stress, aging, and inflammation. In the literature, RDW level has been utilized in the prediction of independent mortality in patients with chronic obstructive pulmonary disease (COPD) acute attack and pneumosepsis. In this study, our primary aim was to evaluate the RDW level in patients with pneumosepsis and COPD acute attack followed up in the ICU. Our secondary aim was to investigate the relation of independent intra-ICU mortality with venous blood gas (VBG ) parameters and age groups. MATERIAL AND METHODS: Patients who were diagnosed with pneumosepsis and COPD acute attack and followed up in the ICU were retrospectively included in the study. Complete blood count (CBC) and VBG values were evaluated both at ICU admission and at 72 hours. RESULTS: A total of 199 participants (45.7% female) with a median age of 74 (range, 20-97) years and who were consecutively admitted to the ICU were included in the study. Participants were classified into three groups according to their sepsis and COPD status: 51 (25.6%) with COPD, 95 (47.7%) with pneumosepsis, and 53 (26.7%) with both COPD and pneumosepsis. Patients with RDW >15% had higher independent intra-ICU mortality, a higher incidence of chronic kidney disease (CKD), lower 72-h HCO3, and higher baseline lactate levels compared to patients with RDW <15%. Patients with intra-ICU mortality were more likely to be older, have higher APACHE II and SOFA scores, and have a higher prevalence of CKD, with statistically significant differences in VBG parameters, including 72-h pH, 72-h PCO2, baseline SO2, 72-h SvO2, baseline HCO3, 72-h HCO3, baseline lactate, and 72-h lactate compared to surviving patients. Moreover, a 19.6% increase in independent intra-ICU mortality risk was detected for each unit increase in RDW after adjustment for confounders. CONCLUSIONS: Based on our findings, we recommend that RDW, HCO3 in VBG, and lactate should be monitored regularly for predicting independent intra-ICU mortality in diseases accompanied with severe inflammation.
