Abstract

OBJECTIVE: In our country, scientific evidence regarding the sociodemographic and clinical characteristics of individuals diagnosed with Selective Mutism (SM) is predominantly limited to case reports. This study aimed to identify and describe the sociodemographic, clinical, and pharmacological treatment characteristics of children diagnosed with SM over a ten-year period in a regional pediatric hospital, in order to contribute systematic clinical data to the limited national literature on this condition. MATERIAL AND METHODS: Medical records of patients aged 1 to 18 years who were diagnosed with SM according to ICD-10 diagnostic criteria and followed at İzmir Dr. Behçet Uz Children’s Hospital over the past ten years were retrospectively reviewed. Relevant variables were recorded using a standardized case report form and evaluated in accordance with predefined inclusion and exclusion criteria. Patients with comorbid psychotic disorders, intellectual disability, and/or developmental delay, autism spectrum disorders, hearing impairments or specific developmental disorders of speech and language were excluded from the study. RESULTS: The study sample consisted of 51 cases. Evaluation revealed no significant gender predominance. Hospital admission rates were found to increase during the school-age period. Social phobia was identified as the most common comorbid psychiatric disorder, present in 23.5% of the cases (n = 12). Regarding pharmacological treatment, 64.7% of the patients (n = 33) were prescribed fluoxetine. The presence of additional psychiatric disorders was associated with longer durations of outpatient follow-up and an increased frequency of medication use (p < 0.05). CONCLUSIONS: The limited scientific data in the field of SM hinders evidence-based inference, thereby reducing the chances of early diagnosis and treatment. Advanced research to be conducted in this area may increase awareness among parents and clinicians, diversify treatment options, and contribute positively to the course of the disorder.
