Abstract

Hemodialysis is one of the essential renal replacement therapies required to sustain life in patients with end-stage renal disease. The success of the treatment is directly associated with the selection and effective management of an appropriate vascular access route. Vascular access is considered the lifeline of hemodialysis; arteriovenous fistula and graft are recommended as the primary permanent access options, whereas central venous catheters are preferred in emergency situations, temporary indications, or when permanent access is not feasible. However, due to the high complication rates of central venous catheter use-including infection, thrombosis, occlusion, bacteremia, and vessel wall damage-international guidelines recommend minimizing catheter utilization, increasing arteriovenous fistula rates, and maintaining chronic central venous catheter use below 10%. With the 2019 Kidney Disease Outcomes Quality Initiative update, the “Fistula First” approach evolved into the “End- Stage Renal Disease Life Plan” model, which prioritizes the patient’s clinical condition, social context, and life goals. This approach emphasizes the importance of individualized treatment planning andconsiders vascular structure, life expectancy, comorbidities, and patient preferences in determining access type. In central venous catheter care, adherence to aseptic techniques, hand hygiene, proper dressing, lumen flushing, heparin locking, and pre-session complication assessment are critical requirements. Nursing care plays a decisive role in central venous catheter management, and patient and family education serves as a complementary factor in reducing infections and other complications. Strengthening multidisciplinary collaboration, implementing early referral strategies, and ensuring adherence to care protocols produce favorable outcomes regarding patient safety, quality of life, and survival rates. This study is a review summarizing nursing care principles related to hemodialysis catheter use in line with current guidelines (Kidney Disease Outcomes Quality Initiative and World Health Organization).

Santral venöz kateter bakımında asepsi-antisepsi uygulamaları, el hijyeni, düzenli pansuman, lümen yıkama, heparinle kilitleme tekniklerine uyum ve her seans öncesi komplikasyon değerlendirmesi kritik gerekliliklerdir. Hemşirelik bakımı santral venöz kateter yönetiminde belirleyici role sahip olup, hasta ve yakınlarının eğitimi enfeksiyon ve diğer komplikasyonların azaltılmasında tamamlayıcı etkidedir. Multidisipliner yaklaşımın güçlendirilmesi, erken sevk uygulamaları ve bakım protokollerine bağlılığın artırılması; hasta güvenliği, yaşam kalitesi ve sağkalım oranları üzerinde olumlu sonuçlar doğurmaktadır. Bu çalışma, güncel kılavuzlar (Kidney Disease Outcomes Quality Initiative ve Dünta Sağlık Örgütü) kılavuzlarına doğrultusunda hemodiyaliz kateter kullanımına ilişkin hemşirelik bakım ilkelerini özetleyen bir derlemedir.
