Abstract

Objective: Nosocomial carbapenem-resistant Gram-negative bacterial infections significantly increase themortality, morbidity, length of stay, and hospitalization costs of patients in intensive care units. Furthermore, treatment options for infections caused by these resistant bacteria are limited, especially in intensive care patients. Ceftazidime-avibactam is a combination drug approved in 2015 and indicated for the treatment of complicated intra-abdominal and urinary tract infections, hospital-acquired pneumonia, and ventilator-associated pneumonia. Our study aimed to investigate the ceftazidime-avibactam susceptibility rates of carbapenem-resistant strains obtained from various clinical specimens in our hospital's intensive care units. Material and Method: This study retrospectively analyzed 171 bacterial isolates grown from intensive careunit patients sent to our laboratory between January 2024 and December 2024. The VITEK2 (bio-Mérieux, France)automated system was used for species identification and antimicrobial susceptibility testing. Ceftazidime-avibactamsusceptibility was determined using the disk diffusion method (Oxoid UK) on Mueller Hinton agar (RTA, Türkiye). Result and Discussion: In the study, resistance to CZA was highest in K. pneumoniae isolates at 24.3%. Forty (23.5%) of all isolates were found to be resistantto ceftazidime-avibactam. Although resistance to this new combination preparation was detected, it remains avery strong alternative, particularly in intensive care patients, as a last resort for the treatment of carbapenem-resistant strains. However, monitoring resistanceprofiles both regionally and globally is crucial.
