Seröz retina dekolmanının yaygınlığının iyot-125 plak radyoterapi uygulamasını takiben arka uvea melanomunun gerilemesi üzerinde olası etkileri

Özet
Amaç: Seröz retina dekolmanı, uvea melanomu olan gözlerde sık rastlanılan bir bulgudur. Bu çalışmada, beş ayrı dekolman tipinde, retina dekolmanının yaygınlığının, 1-125 brakiterapi-sini takiben üvea melanomu gerilemesi üzerindeki olası etkileri incelenmiştir.Yöntem: Uvea melanomu olan ve 1-125 plak brakiterapi ile tedavi edilen 35 göz, tip 0: klinik olarak belirgin retina dekolmam yok, tip 1: 1 kadranda lokalize dekolman, tip 2: 2 kadranda dekolman, tip 3: tümörü çevreleyen dekolman ve tip 4: total retina dekolmanı olarak sınıflandırıldı. Yerel tümör kontrolü, ultrasonografik tümör kalınlığı ölçümü ile değerlendirildi. Sonuçlar: Ortalama takip süresi 34.5 aydır. Postoperatif yaklaşık 20. aya kadar, grup 2'de-ki tümör gerilemesi, grup 0 ve 1'e göre anlamlı olarak daha yavaştır (p=0.031 ve p=0.044). Daha sonra tüm gruplarda benzer oranlarda anlamlı gerileme gözlenmiştir. Tartışma: Sınırlı sayıda hasta içeren çalışmamıza göre, arka üvea melanomu ile birlikte seröz retina dekolmanı, yaygınlığına bakılmaksızın, tümör yüksekliğinde azalma ile ifade edilen yerel tümör kontrolünde belirleyici bir etken değildir.
Abstract
Purpose: Serous retinal detachment is a common finding in eyes with uveal melanoma. In this study, possible effects of retinal detachment on the regression of uveal melanoma after I-125 braehytherapy are explored by comparing five arbitrarily chosen patterns of detachment. Methods: 35 eyes treated with braehytherapy for posterior uveal melanoma were classified as follows: type 0: no associated retinal detachment, type 1: localized detachment in 1 quadrant, type 2: detachmant in 2 quadrants, type 3: detachment surrounds the tumour and type 4: total retinal detachment. The outcome of local tumour control was assessed by ultrasonographic tumor thickness measurements. Results: The mean follow-up was 34.5 months. Until approximately 20 months postoperatively, tumour regression was significantly slower in group 2 compared to groups 0 and 1 (p=0.031 and p=0.044 respectively). After then, there was no difference between groups and all tumours regressed significantly and similarly. Conclusion: Our data on a limited number of patients suggest that the presence of serous retinal detachment in association with posterior uveal melanoma and its extent and severity is not a determinant factor in the local tumour control in terms of reduction in tumour height.
