Özet

Amaç: Fakoemülsifikasyon ve göz içi lens implantasyonu cerrahisinde topikal anestezinin etkinliğini, güvenilirliğini ve hasta memnuniyetini incelemek. Yöntem: Topikal anestezi altında fakoemülsifikasyon ve göz içi lens (GİL) implantasyonu uygulanan yaşları 21-80 (ort.62,3 ± 7,7) arasında değişen 85 hastanın (45-kadın, 40-erkek) 105 gözü prospektif olarak değerlendirildi. Hastaların gözlerine ameliyat başlamadan 10-5 dakika ve ameliyattan hemen önce olmak üzere %0,05'lik proparacaine HCl içeren damla damlatılarak cerrahi uygulandı. Ameliyat sırasında ve sonrasında hastaların ağrı dereceleri sorgulandı. Sonuçlar: Fakoemülsifikasyon aşamasında hastaların %14,3'ü hafif ağrı, %4,2'sı orta şiddetli ağrı, lens implantasyonu aşamasında ise %9,5'i hafif ağrı, %3,8'i orta derecede ağrı bildirirken, hiçbir hasta şiddetli ağrı bildirmedi. Aşırı glob hareketi %14,3 gözde izlendi. Olguların hiçbirinde arka kapsül yırtılması ve vitreus kaybı gelişmedi. Hastaların %78,8'i çok memnun, %15,2'i memnun, % 5.8'i memnun olmadığını bildirdi. Tartışma: Dikkatli hasta seçimi ile deneyimli bir cerrah tarafından topikal anestezi altında uygulanan fakoemülsifikasyon ve GİL implantasyonu etkin ve güvenilir bir yöntemdir. Rahatsız edici olmaması ve hasta memnuniyeti açısından tercih edilebilir.

Abstract

Purpose: To evaluate the efficacy, safety and patient satisfaction of topical anesthesia in phacoemulsification and intraocular lens (IOL) implantation. Method: 105 eyes of 85 patient (45 female, 40 male) who underwent phacoemulsification and IOL implantation under topical anesthesia were evaluated prospectively. Mean age of the patients was 64,3± 7,7 years (range 21-80). %0.05 proparacaine HCL drop was administered to eyes of the patients 10 and 5 minutes prior to surgery and just before the beginning the surgery. All the patients were asked to evaluate pain and discomfort during and after the surgery. "Results: During phacoemulsification phase %14,3 of the patients reported mild pain, %4,2 reported moderate pain. Mild pain was reported by %9,5 of the patients and moderate pain by %3,8 during IOL implantation. No severe pain was reported at any stage. Exessive ocular moti-lity was observed in %14,3 of the cases. Posterior capsule perforation or vitreus loss did not oc¬cur in any case. %78,8 of the patients were very satisfied, %15,2 were satisfied and %5,8 were not satisfied. Conclusion: Phacoemulsification and IOL implantation under topical anesthesia by an ex¬perienced surgeon with appropriate patient selection is an efficient and safe procedure. Topical anesthesia; being noninvasive with high patient satisfaction can be the choice of anesthesia in phacoemulsification and IOL implantation.
