Özet

Çocuklarda semptomatik hepatitA virüs (HAV) infeksiyonunun klinik ve biyokimyasal özellikleri ile atipik klinik seyir gösteren olguların incelenmesi amaçlanmıştır. Yöntem: Akut bepatit belirtileri ile başvuran ve anti-HAV IgM pozitifliğinin gösterilmesi ile bepatitA tanısı alan 95 hastanın klinik ve laboratuvar verileri retrospektif olarak incelenmiştir. Bulgular: En fazla (%56.8) hasta 6-10 yaş arasında bulundu. Hepatitli hasta ile temas öyküsü sadece 15 (%15.7) hastada saptandı. Hastaların %70.5'inde sanlık, %54.7'sinde hepatomegali ve 9510.5' inde splenomegali saptandı. Laboratuvar testlerinde aspartat aminotransferaz (AST) 25-4854 U/L, alanın aminotransferaz (ALT) 5-4574 U/L, alkalen fosfataz (ALP) 97-2515 U/L total bilirubin 0.47-28.33 mg/dL arasında bulundu. Atipik belirtiler bir hastadafulminan hepatit, bir hastada alevlenme gösteren klinik seyir, bir hastada kolestaz ola-rak belirlendi, îkisindeplevral efüzyon da bulunan 3 hastada asit saptandı. Beş (%5.2) hasta hastaneye yatırılarak, 90 (%94.7) hasta ayaktan izlendi. Yorum: HepatitA infeksiyonunun çocuklarda genellikle komplikasyonsuz seyretmesine rağmen, fulminan hepatik yetmezliğe, kolestaza, plevral efüzyon ve asile de neden olabileceği sonucuna varılmıştır.

Abstract

Objective: The purpose of this study was to determine the clinical, biochemical features and atypical manifes-tations of symptomatic hepatitis A virus (HAV) infections in children. Method: Ninety-five patients with symptoms of acute hepatitis and confinned diagnosis of hepatitis A by the de-tection of anti-HAV IgM were studied in this retrospective study. Results: The majority of patients (56.8%) were in age group of 6-10 years old. A history of previous exposure to a patient with hepatitis was elicited in 15/95 (153%). Seventy percent of patients had jaundice hepatomegaly was present in 54.7% and splenomegaly 10.5%. The results of laboratory tests were aspartate aminotransferase (AST) 25-4854 U/L, alanine aminotransferase (ALT) 5-4574 U/L, alkaline phosphatase (ALP) 97-2515 U/L, total bilirubin levels 0.47-28.33 mg/dL. Atypical manifestations induced fulminant clinical sourse, relapse and cholestasis Three patient spresented with ascites, two of them had pleural effusions Of these 95 patients five (5.2%) were admitted to the hospital. Conclusion: We concluded that the course of PIAV infections in children is usually uncomplicated, but HAV is also associated with fulminant hepatic failure, cholestasis, pleural effusion and ascites.
