Özet
Çocukluk çağında nadir görülen myelodisplastik sendromlar (MDS) genellikle kötü prognoza sahiptir. 1989-1996 döneminde merkezimizde medyan yaşlan 7.5yıl olan (1.8-17yaş) 8primer MDS olgusu iz-lendi. FAB kriterlerine göre 4'ü refrakter anemi (RA), 4'ü blast artışı ile refrakter anemi (RAEB) tanısı alan olguların 7'si, median 6 ayda (2 ay-2yıl) ANLL'ye transmisyon gösterdi. RA'li olgulardan biri he-molitik anemi ile başvurmuş, diğerinde ise myelofibrozis saptanmıştır. AML kenioterapisi uygulanan 6 hastadan 2'sinde tam, 2'sinde parsiyel remisyon sağlanabilmiştir. Bu hastalardan 2'sine allojenik ke-mik iliği nakli uygulanmış, ancak biri sistemik mantar enfeksiyonuyla, diğeri erken relaps ile kaybedil-miştir. Yaşayan tek hasta, doku grubu uygun kardeşi olmadığından, RAEB olarak 1.5yıldır destekleyi-ci tedaviler ve kemoterapi ile izlenmektedir. Kemik iliği nakli yapılan iki olgumuzun da kaybedilmesi-ne karşın, kemik iliği naklini RA veRAEB'lerde tedavi edilmeden, RAEB-t veANLL'lerde indüksiyon son-rası tam ya da parsiyel remisyonda ilk tedavi seçeneği olarak görmekteyiz. Uzun süreli kemoterapiler organ toksisi(elerine neden olarak kemik iliği naklinin başarısını olumsuz yönde etkilemektedir.
Abstract
Myelodysplastic syndromes (MDS) are rarely observed in children and mostly have a poor prognosis. We describe the clinical characteristics and outcome of 8 children with primary MDS (median age 7.5 ye-ars, range 1.8 - 17) followed between 1989-1996. Four patients had refractory anemia (RA), four ref-ractory anemia excess blast (RAEB) at presentation and seven of them developed acute nonlymphoid le-ukemia CANLL) within median 6 months (range 2 months - 2 years). One of the patients with RA pre-sented with hemolytic anemia and another had myelofibrosis. Out of six patients, two partial and two complete remissions were achieved after AMI chemotherapy (CT). Two of these patients underwent allo-geneic bone marrow transplantation (allo BMT) but one died with systemic fungal infection and the ot-her with early relapse. The only surviving patient in RAEB without any compatible sibling donor is un-der follow-up for 1.5 years with supportive measures and chemotherapy. Although our patients who un-derwent allo BMT were not long term survivors we still consider Allo BMT as a therapy of first choice for untreated RA, RAEB and refractory anemia with excess blast in transformation (RAEB-t) andANLL af-ter induction of complete or partial remissions as long term chemotherapy might cause severe organ to-xicities.
