Özet

Bu çalışmanın amacı, toplum kaynaklı 67 akut pürülan meninjit olgusunu retrospektif olarak değerlendirmek idi. Hastalar en sık başağrısı (%79.1) ve bilinç değişimi (%73.1) ile başvurmuştu ve en sık saptanan bulgular meningeal irritasyon bulguları (%89.6) ve bilinç değişimi (%73.1) idi. Altta yatan hastalık (%31.3), daha önce antibiyotik kullanımı (%22.4), kafa travması öyküsü (%22.4), beyin cerrahi girişimi (%6) ve immünosupresif tedavi (%3) meninjit gelişmesinde yardımcı faktörler olarak bu/undu. Sıklıkla Streptococcus pnumoniae (%60) ve Neisseria meningitidis (%26.7) etken olarak izole edildi. En sık kullanılan antibiyotikler penisilin G ve üçüncü kuşak sefalosporinlerdi. Hastaların %46.3'üne antibiyotik tedavisine ek olarak destekleyici tedavi verildi. Mortalite oranı %34.3 olarak saptandı. Bilinç bozukluğu, periferik kanda lökositoz ve trombositopeni kötü prognostik faktörler olarak bulundu.

Abstract

The purpose of this study was to retrospectively evaluate 67 community acquired-acute purulent meningitis cases. In the cases, headache (79.1%), mental status change (73.1%) and meningeal irritation signs (89.6%) were the most common signs and findings. Predisposing factors for meningitis were; underlying disease (31.3%), previous antibiotic usage (22.4%), previous head trauma (22.4%), previous neurosurgical intervention (6%), and immunosuppressive treatment (3%). The most common pathogens were Streptococcus pneumoniae (60%) and Neisseria meningitidis (26.7%). For the treatment; penicillin G and third generation cephalosporins were commonly used. Adjunctive therapy was used in 46.3% cases. Mortality rate was 34.3%. Mental status change, leukocytosis and thrombocytopenia were poor prognostic factors for acute purulent meningitis.
