Özet

Yaşlı hastalarda cerrahi tedavi riskli olmasına rağmen ileri yaş cerrahiye kontraindikasyon oluşturmaz. Yaşlılar¬da laparoskopik işlemler açık ameliyatlara göre daha iyi sonuçlar vermektedir. Kolelitiazis insidensi yaşla birlikte artmaktadır. Kolesistektomi yaşlı hastalarda sık uygula¬nan bir işlemdir. 1995 yılında cerrahi kliniğimizde safra kesesi taşı nedeniyle 558 hastaya kolesistektomi uygulan¬dı. 381 hastaya laparoskopik kolesistektomi (LK) ve 177 hastaya açık kolesistektomi (AK) uygulandı. Grup l'de (65 yaş ve üzeri LK) 22 erkek, 59 kadın; Grup 2'de (65 yaş altı LK) 62 erkek, 238 kadın; Grup 3'de (65 yaş ve üzeri AK) 20 erkek, 32 kadın vardı. 65 yaş ve altı 125 AK olgusu mevcut idi ve çalışmaya alınmadı. Ortalama yaş Grup l'de 69.55±3.2, Grup 2'de 41.53±10.3 ve Grup 3'de 70.50+4.1 idi. Preoperatif medikal morbidite oranı sıra¬sıyla % 76.54+16.46, % 33+3.5 ve % 76.92±20.5 idi. Açı¬ğa dönüş oranı Grup l'de % 6.17 ve Grup 2'de % 3.36 idi. Postoperatif morbidite sırasıyla % 23.4+4.9, % 4+0.39 ve % 59.6+15.8 idi. Hastanede kalış süresi sırasıyla 3.5±0.2 gün, 2.74±1.2 gün ve 5.93±0.6 gün idi. Sadece Grup 3'de postoperatif 2 hasta (Myokard enfarktüsü ve safra kaçağı) mortal seyretti (% 3.84±0.88). Bu çalışma yaşlı hastalar¬da uygulanan laparoskopik kolesistektominin postoperatif komplikasyon ve hastanede kalış süresi açısından açık kolesistektomiye göre daha üstün olduğunu göstermekte¬dir. Genç hasta grubuna göre yaşlılarda medikal prob¬lemler morbidite ve mortalitede etkin gözükmektedir.

Abstract

Laparoscopic Cholecystectomy in the Elderly Although the surgical treatment in elderly patients is asso¬ciated with increased risk, advanced age is no longer a contraindication to surgery. Laparoscopic procedures in the elderly have better results than open surgery. The inci¬dence of cholelitiasis has been shown to increase with ad¬vancing age and thus cholecystectomy is a common opera¬tion in elderly patients. 558 patients underwent cholecystectomy for gallstone disease at our surgical de¬partment during 1995. Laparoscopic cholecystectomy (LC) and open cholecystectomy (OC) was performed in 381 and 177 patients respectively. There were 22 men, 59 women in group 1 (>65 years oldLC); 62 men, 238 women in group 2 (65 years old OC). There were 125 patients younger than 65 years old in OC group and not included in the study. The mean age was 69.55+3.2 in group 1, 41.53110.3 in group 2, and 70.50±4.1 in group 3. The rate of preoperative medi¬cal morbidity was % 76.54+16.46, % 33±3.5 and % 76.92+20.5 in group 1,2,3 respectively. The rate of con¬version to ac was % 6.17 in group 1, and % 3.36 in group 2. The postoperative morbidity rate was % 23.4+4.9, % 4+0.39, % 59.6±15.8 in group 1, 2 and 3 respectively. The mean length of postoperative hospital stay was 3.5+0.2 days in group 1, 2 74±1.2 days in group 2 and 5.93±0.6 days in group 3. Two patients died (Myocardial infarction and bile leak) postoperatively for a mortality rate of % 3.84±0.88, only in group 3. This study indicates that LC can be performed in the elderly patients with postoperative complications and hospital stay results superior to those re¬ported for elderly patients undergoing OC. The effects of medical problems on morbidity and mortality are the risks of elderly age group compared to younger age group.
