Özet

Postoperatif hipokalsemi, tiroid cerrahisi sonrası nadir görülen bir komplikasyondur. Paratiroid bezlerin hasan, devaskülarizasyonu veya rezeksiyonu en sık nedenleridir, fakat operatif manüplasycfni&Kâsıflda kalsitonin sahilimi ve tirotoksik osteodistrofiniW'{p^istoperatif düzelmesine bağlı oluşan "aç kemik sendromu" gibi diğer nedenler de akılda bulundurulmalıdır. Çalışmanın amacı, tiroid cerra¬hisi sonrası oluşan hipokalsemi insidansı ve risk faktörle¬rini klinik deneyimlerimiz ve literatür ışığı altında değer¬lendirmektir. Çalışmamızda Ocak 1993-Ocak 1997 tarih¬leri arasında AnabiUm Dalımız'da öpere edilen 382 olgu¬nun prospektif değerlendirilmesi yapılarak, postoperatif dönemde oluşan hipokalsemiyi etkileyen faktörler araştı¬rılmış ve bu komplikasyonların azaltılması için başvurula¬bilecek yöntemler ortaya konulmaya çalışılmıştır. Lobek-tomi sonrası oluşan hipokalsemi ile yaş, tiroid lokalizas-yonu, tiroidin fonksiyonel durumu, tiroidektomi materyali¬nin histopatolojik bulguları, ilk veya ikincil ameliyatlar ve uygulanan ameliyat tipleri arasındaki ilişkiler; univariate (chi kare, fisher kesin olasılık testi) analiz yöntemleri ile araştırılmıştır. Serimizde lobektomi yapılan 7 (% 1.8) ol¬guda klinik olarak hipokalsemi bulguları saptanmıştır. Ol¬gulardan 6 (% 1.5)'sında geçici, 1 (% 0.3)'inde kalıcı hi¬pokalsemi gelişmiştir. Abstract

The Causes of Hypocalcemia Following Thyroid Surgery Postoperative hypocalcemia is a relatively common com¬plication following thyroidectomy. Damage, devasculari-zation, or inadvertent removal of the parathyroid glands is the most widely accepted reason, but other causes have been postulated including release of calcitonin during op¬erative manuplation and hungry bone syndrome due to postoperative reversal of thyrotoxic osteodistrophy. The aim of this study is to evaluate the incidence and risk fac¬tors of hypocalcemia following thyroidectomy with the help our clinical experiences and the literature. In this study, 382 cases who underwent thyroidectomy were ana¬lyzed for factors increasing the risk for postoperative hy¬pocalcemia prospectively. The statistical analysis between hypocalcemia and age, the disease process and the locali¬zation and the status of the thyroid fuction, histopatholog-ic data of thyroidectomy materials, primar, or secondary operations, and the performed procedures were performed by using chi square and Fischer exact tests. Clinically ob¬served hypocalcemia was seen in 7 (% 1.8) of the patients, 6 (1.5 %) of them being temporary.
