Primer akciğer kanseri ve beyin metastazı (57 olgunun retrospektif analizi)

Özet
Kliniğimizde 1997-2000 yılları arasında primer akciğer kanseri tanısı almış olgulardan, beyin metastazı saptanan 57 olguyu retrospektif olarak değerlendirdik. Olguların 56'sı erkek biri kadındı ve yaş ortalaması 59.49 ± 10.66 yıl idi. Yirmiyedi (%47.3) olguda tanı anında beyin metastazı vardı ve bunlardan üçü beyin operasyonu geçirmişti. Onbir (%19.2) olgu asemptomatikti, en sık semptom baş dönmesi (28 olgu, %49.1) ve baş ağrısıydı (27 olgu, %47.3). Akciğer kanseri histopatolojik dağılımı; adenokarsinom 28 (%49.1), skuamöz hücreli karsinom 17 (%29.8) ve küçük hücreli karsinom 12 (%21) şeklindeydi. Akciğer tümörlerinin %57.8'i periferik yerleşimliydi. Beyin metastazları sıklıkla N2 hastalık (31 olgu, %54.3) ve T3 (29 olgu, %50.8) ile birlikteydi. Tanı konduktan sonra ortalama beyin metastazı gelişme süresi 50.6 gün (15-105) idi. En sık metastaz şekli soliter tipte metastaz (n= 37, %64.9) ve en sık metastaz lokalizasyonu pariyetal (n= 22, %38.5) lobdu. Olguların tümüne kranial radyoterapi uygulanmış ve semptomlar kontrol altına alınmıştı. Median sağkalım süresi akciğer kanseri tanısından sonra 120 (94-145) gün, beyin metastazından sonra 90 (63-116) gün bulundu. Beyin metastazı sonrası sağkalımda etkili Prognostik faktör yalnızca nörolojik semptomların olup olmaması (p= 0.0071 ) iken, akciğer kanseri sonrası sağkalımda etkili bulunan faktörler; beyin metastazının senkron-metakron oluşu (p= 0.0002), N durumu (p= 0.0492) ve olgulara kemoterapi (KT) uygulanıp uygulanmaması (p= 0.0007) idi. Sonuç olarak; çalışmamızda en sık periferik yerleşim ve histopatolojik olarak adenokanser saptandı. Radyoterapiye rağmen median sağkalım süresi oldukça kısa idi.
Abstract
We evaluated retrospectively 57 cases diagnosed as primary lung cancer with brain metastases in our clinic between 1997-2000. Fifty six of cases were male, one was female and the mean age was 59.49 ± 10.66 years. Twenty seven cases (47.3%) had brain métastases at the time of diagnosis and three of them had been operated. Eleven (19.2%) cases were asymptomatic; the most common symptoms were dizziness (28 cases, 49.1 %) and headache (27 cases, 47.3%). The histopathological distribution was as following; adenocarcinoma 49.1% (n= 28), squamous cell carcinoma 29.8% (n= 17), small cell carcinoma 21% (n= 12). Peripherie localization was determined in 57.8% of lung tumors. Brain métastases were usually with N2 disease (31 cases, 54.3%) and T3 tumor (29 cases, 50.8%). The mean metastasing time was 50.6 days (15-105) after diagnosis. The most frequent metastasing form was solitary metastasis (n= 37, 64.9%), the most frequent metastasing site was parietal lobe (n= 22, %38.5). All patients had received cranial radiotherapy and symptoms had been controlled. The median survival time of patients was 120 (94-145) days after diagnosis of lung tumor and it was 90 (63-116) days after diagnosis of brain metastasis. Although the presence of neurologic symptoms (p= 0.0071 ) is the only prognostic factor affecting survival after brain metastasis, the factors affecting survival after establishing diagnbsis of lurig'cancer are; the status of brain metastasis-synchronous or metachronous (p= 0.0002), N status (p= 0.0492) and whether the patients received chemotherapy or not (p= 0.0007). Consequently, in our study, peripherie localization and histopathologically adenocarcinoma were mostly determined. Although, all patients had received RT, the median survival time was quite short.
