Helikobakter pilori eradikasyonunun fonksiyonel dispepsili vakalarda klinik semptomlar, serum gastrin ve pepsinojen I üzerine etkisi

Özet
Fonksiyonel dispepsi etiopatogenezinde Helikobakter pilori (Hp) enfeksiyonunun rol oynadığı düşünülmektedir. Hp eradikasyon tedavisinin klinik semptomlar üzerindeki etkisiyle ilgili tartışmalı sonuçlar bildirilmiştir. AMAÇ: Çalışmamızın amacı Hp pozitif fonksiyonel dispepsili bireylerde eradikasyon tedavisinin klinik semptomlar, serum gastrin ve pepsinojen I üzerindeki etkisini araştırmaktı. GEREÇ VE YÖNTEM: Çalışmaya Hp (+) fonksiyonel dispepsi tanısı konulan, yaş ortalaması 28 ±8 olan 40 hasta (18 erkek, 22 kadın) alındı. Birinci gruba (n=20) Ranitidin bizmut sürat 2x400 mg/gün + Klaritromisin 2x500 mg/gün ve ikinci gruba (n=20) Lansoprazol 2x30 mg/gün + Klaritromisin 2x500 mg/gün + Amoksisillin 2x1000 mg/gün 14 gün verildi. Tedavi öncesi, tedavi bitiminde ve tedaviden 6 ay sonra semptom skoru, Hp varlığı, serum gastrin ve pepsinojen I düzeyi saptandı. SONUÇLAR: Birinci gruptaki 16 (%80) ve ikinci gruptaki 13 olguda (%65) olmak üzere toplam 29 (%73) olguda eradikasyon sağlandı. Hem Hp eradikasyonu sağlanan hem de sağlanmayan olgularda tedavi bitiminde ve tedavi bitiminden 6 ay sonra tedavi öncesine göre ortalama semptom skorunda anlamlı azalma saptandı (p<0.001). Hem eradikasyon sağlanan hem de sağlanmayan grupta, serum gastrin düzeyi tedavi öncesine göre tedavi bitiminde anlamlı değişmediği halde, tedavi bitimimden 6 ay sonra anlamlı olarak azaldı (sıra ile p<0.001, p<0.05). Eradikasyon sağlanan grupta serum pepsinojen I düzeyinde tedavi öncesine göre tedavi bitiminde anlamlı değişme olmadığı halde, tedavi bitiminden 6 ay sonra anlamlı artma oldu (p<0.05); eradikasyon sağlanamayan grupta ise takip boyunca anlamlı değişme olmadı. TARTIŞMA: Sonuç olarak Hp (+) fonksiyonel dispepsili olgularda, eradikasyon tedavisi Hp eradikasyonuna bağlı olmaksızın, semptomlarda uzun süreli düzelme sağlarken serum gastrin ve pepsinojen I üzerinde belirgin etki etmemektedir.
Abstract
Helicobacter pylori (Hp) is considered to be important factor in the etiopathogenesis of functional dyspepsia. There are conflictive results about eradication treatment ofHp on clinical symptoms. AIM: The aim of our study was to determine the effects of eradication of Hp on clinical symptoms, serum gastrin and pepsinogen 1 levels. MATERIALS AND METHOD: A total of 40 patients (18 male, 22 female) with functional dyspepsia, mean age of 28 ± 8 years, .were included to the study. Ranitidine bismuth citrate 400 mg BID + Clarithromycin 500 mg BID were given'to 20patients (group 1) and Lansoprasole 30 mg BID, Clarithromycin 500 mg BID + Amoxicilline 1000 mg BID were given to 20 patients (group 2) for 14 days. Symptoms score, serum gastrin and pepsinogene I level were identified for all patients before, just completion and 6 months after completion of treatment. RESULTS: Hp eradication was provided in a total of 29 (73 %) including 16 (80 %) patients of group 1 and 13 (65 %) patients of group 2. In both patients in which eradication ofHp was provided (HP (+)) and not provided patients (Hp (-)) , there were significant improvement in symptoms score during completion and 6 months after completion of treatment compared to before treatment (p<0.001). In both Hp eradicated and not eradicated patients, there was not significant change in serum gastrin level during completion of treatment compared to before treatment, but there was significant change 6 months after treatment compared to before treatment (p<O.OOJ, p<0.05; respectively). In the Hp (+) patients, serum pepsinogen I level did not change significantly during completion of treatment compared to before treatment, but increased significantly 6 months after completion of treatment compared to before treatment (p<0.05). In the Hp (+) patients, there was not significant change in pepsinojeri I level during follow-up. CONCLUSIONS: In Hp positive patients with functional dyspepsia, in contrast to eradication treatment for Hp provides improvement in clinical symptoms for a long time independing eradication ofHp; does not effect on serum gastrin and pepsinogen I levels.
