Özet

Nüfusun hızla yaşlanması, uzun süreli bakım ihtiyacını kritik bir sosyal politika meselesi haline dönüştürmüştür. Bu çalışmanın amacı, OECD ülkelerindeki uzun süreli bakım sistemlerini ampirik verilerle farklı tipolojiler altında sınıflandırmak ve kurumsal bakım sigortası bulunmayan Türkiye’nin bu tipolojilerdeki konumunu değerlendirmektir. Araştırmada 15 OECD ülkesinin 2021 yılına ait yedi sosyal politika göstergesi incelenmiş; iki aşamalı kümeleme (Ward ve K-Ortalamalar) ile ANOVA testleri uygulanmıştır. Bulgular, bakım rejimlerinin üç farklı tipolojide kümelendiğini göstermektedir: Karma/Geçiş Tipi Sistemler, kamusal müdahalelere rağmen bakım yükünü cinsiyet rolleri üzerinden aileye bırakmaktadır. Toplumsallaşmış Sistemler, güçlü kamu finansmanı ve kurumsal hizmetlerle bakımı profesyonelleştirerek aileden bağımsızlaştırmaktadır. Aile Temelli ve Finansal Baskı Altındaki Sistemler ise kapasite yetersizliği nedeniyle yeniden aileleşme eğilimi göstermektedir. Rejimleri ayrıştıran temel faktörün harcama miktarından ziyade kaynak kullanım stratejisi olduğu saptanmıştır. Birinci ve üçüncü kümeler arasında sarkaç görünümü çizen Türkiye’nin kurumsal kapasite krizini önlemek adına, bakım hizmetlerini profesyonelleştirmesi ve Uzun Süreli Bakım Sigortası altyapısını ivedilikle inşa etmesi gerekmektedir.

Abstract

The rapid aging of the population has made the need for long-term care a critical social policy issue. The aim of this study is to classify long-term care systems in OECD countries under different typologies using empirical data and to assess Turkey’s position within these typologies, given that it lacks an institutional long-term care insurance system. The study examined seven social policy indicators for 15 OECD countries as of 2021; two-stage clustering (Ward and K-means) and ANOVA tests were applied. The findings indicate that care regimes cluster into three distinct typologies: Hybrid/Transitional Systems, which, despite public interventions, leave the burden of care to the family through gender roles. Socialized Systems professionalize care through strong public funding and institutional services, thereby decoupling it from the family. Family-Based and Financially Strained Systems, however, exhibit a tendency toward re-familialization due to capacity shortages. It has been determined that the key factor distinguishing these regimes is not the amount of spending but the strategy for resource allocation. To prevent an institutional capacity crisis, Turkey—which exhibits a pendulum-like oscillation between the first and third clusters—must professionalize care services and urgently establish the infrastructure for Long-Term Care Insurance.
