Cholecystectomy through mini-laparotomy and laparoscopic cholecystectomy

Abstract

Cholecystectomy through mini-laparotomy has been re¬ported as a minimally invasive procedure since 1982. Since Jan. 1$93 we perform cholecystectomy through min-ilaparotomy for patients unable to pay for the laparoscop-ic cholecystectomy which we perform since May 1992 at our clinic. In two cases of acute and 35 cases of chronic cholecystitis with stone formation, cholecystectomy through minilapa-rotomy was performed with a 95 % success rate. Thirty one patients were female, 6 patients were male (mean age and weight 37 years and 57.5 kg respectively).. In two cas¬es conversion to classical laparotomy was necessary (5 %). Mean operation time was 52 (35-95) min. 70.3 % of patients were discharged postoperative 48 h later and again 73.3 % of patients returned to their normal activity at the and of two weeks after operation. A comparison has been made with the results of 102 pa¬tients who underwent laparoscopic cholecystectomy. In conclusion, cholecystectomy through mini-laparotomy is a minimally invasive procedure, which is less painfull, yields better cosmetic results and necessitates less hospi¬tal stay and operating time than the conventional method. It can be used conveniently in cases where laparoscopic cholecystectomy cannot be performed either due to patient preferance and economical reasons.

Minimal invaziv bir girişim olarak, mini-laparotomik ko-lesistektpminin 1982 yılından beri uygulandığı bilinmekte¬dir. Biz de Mayıs 1992 'den itibaren ilk tercih edilmesi ge¬reken yöntem olarak uyguladığımız laparoskopik kolesis¬tektomi yanında Ocak 1993'den itibaren laparoskopik gi¬rişimi kabul etmeyen, rastgele seçilmiş hastalara mini-la¬parotomik kolesistektomi uygulamaya başladık. İkisi akut taşlı kolesistit, 35'i kronik taşlı kolesistit bulgu¬ları gösteren 31 'i kadın, 6'sı erkek hastada (ortalama yaş 37, ortalama ağırlık 57.5 kg) % 95 başarıyla uygulandı. İki hastada klasik laparotomi insizyonuna geçildi (% 5). Ortalama operasyon süresi 52 (35-95) dk. oldu. Hastala¬rın % 70.3'ü iki günde taburcu edildiler ve yine hastaların % 73.3'ü 2 hafta içinde normal aktivitelerine döndüler. Elde edilen sonuçlar 102 olguluk laparoskopik kolesistek¬tomi serimizle karşılaştırıldı. Sonuç olarak, mini-laparotomik kolesistektomi kısa ope¬rasyon süresi, iyi kozmetik görünüm, konvansiyonel yönte¬me göre daha az ağrı ve kısa hospitalizasyon süresi ile mi¬nimal invaziv bir girişimdir. Laparoskopik kolesistektomi-nin uygulanamadığı (tercih veya maliyet nedeniyle) hasta¬lara emniyetle uygulanabilir.
