Abstract

Two hundred and thirty patients were admitted to our clinic with splenic injuries, between 1998 and 2003. 168 (73%) patients were male, 62 (27%) patients were female. The average age of patents were 29 years. There were 191 (83%) blunt and 43 (18%) penetrating injuries. The management of our cases involved splenectomy in 135, splenoraphy in 47 and partial splenectomy in 20 and 28 patients were treated conservatively. There were concomitant organ injuries in 152 (66%) patients. Complication rate after splenectomy was 17%, after splenoraphy 5% and in conservatively treated cases it was 3,5%. Mortality was 8,7% (20 patients) and they were all splenectomized patients having RTS less than 2. Splenectomized patients having isolated spleen injury were hospitalized 6 days, patients treated by splenoraphy this is about 7,1 days. Patients having concomitant organ injury were hospitalized approximately 14 days. In conservatively treated patients average hospitalization was 10 days.

Özet

Kliniğimize 1998-2003 yılları arasında 230 dalak travmalı hasta başvurmuştur. Hastaların 168'i (%73) erkek, 62'si (%27) kadındır. Hastaların yaş ortalaması 29'dur. Hastalardaki dalak yaralanması 191 (%83) tanesinde künt, 43 (%18) tanesinde penetran karın yaralanmasına bağlı idi. Dalak travmalı hastaların 135'ine splenektomi, 47'sine splenorafi, 20'sine kısmi splenektomi yapıldı. Yirmisekiz hastaya konservatif yaklaşım uygulandı. Hastaların 152'sinde (%66) yandaş organ yaralanmaları mevcuttu. Ameliyat sonrası komplikasyon oranı splenektomilerde %17, dalak koruyucu girişimlerde %5, konservatif kalınanlarda %3.5'tur. Toplam 20 hasta kaybedilmiştir (%8,7) ve bunların hepsi değiştirilmiş travma skoru (RTS) 2'nin altında splenektomize hastalardır, izole dalak travmalı hastalardan splenektomi yapılanların ortalama hastanede kalış süreleri 6 gündür. Dalak koruyucu işlem yapılanlarda bu süre 7.1 gündür. Dalak ile birlikte yandaş organ yaralanması olanların hastanede kalış süreleri ortalama 14 gündür. Konservatif tedavi edilenlerde bu süre 10 gündür.
