Abstract

Tendon Lengthening Via Incomplet Horizontal Cuts; A Biomechanical Study Commonly employed tendon elongation process in transecting the tendon in a step-like fashion and primarily repairing it following advancement (Tendon Z-Plasty). There are other techniques such as dividing the tendon from the musculotendineous junction, sagitally splitting the tendon and suturing the free half of the tendon to the original insertion, or elongation with multiple incomplete horizontal notching. Feet of freshly slain chicken were used in our study in which modifications in tendon elongation via incomplet horizontal cuts were applied. Incomplete transverse incisions were applied on the middle 2/4 of the tendon; 3 on one side, 2 on the other. Each incision was 2.5 mm in depth, crossing the midline by 0.5 mm. summing 1 mm from each cut, a total 5 mm of elongation was obtained. In the Z-plasty group, the length of the step-cut was designated to be 0.5 cm, resulting in a elongation of 1 cm. Test of tensile strength was carried out via a mechanical testing deviceat a distraction rate of 20 mm/sec. Intact tendons withstood a mean tension of 556.9 N (450-635). Z-plasty and trial groups withstood a mean 44.1 N (29-52) and 39.4 N (17-98) respectively. Results rervealed no statistical significance. The modifications we made to tendon elongation via incomplete incisions proved to have advantages such as not disrupting tendon continuity and not significantly compromising the tendon's tensile strength. This report suggests that, if combined with a reinforcing 6/0 continious epitendinous suture for the notch edges, this technique is clinicaly applicible as a method obviating the need for splinting, allowing immediate motion, and being less prone to adhesions due to the absence of a core sutre and bulky of knot.
