Abstract

Low grade glioma (LGG) is a clinical definition that has originated from clinical modification of a pathological diagnosis. Tumors defined by this term vary according to anatomical location, clinical behavior and response to treatment. This review aims to define the historical evolution and the shortcomings of the LGG term, define clinically relevant molecular subsets and propose an alternate classification. Benign and malignant forms of brain tumors were known since 1863. Through the years several unique tumor biologies were defined and excluded from this classification. Novel molecular entities have defined further clinically relevant subsets and have made LGG definition obsolete. The LGG term harbors astrocytomas, oligodendrogliomas and other tumors that mimic these two entities (glioblastomas, expansile gliomas etc). Molecular markers have enabled a clinically much more relevant definition of gliomas.
