Abstract

Objective: The aim of this study was retrospectiveinvestigation of the use of non-carbapenem antibiotics asoral sequential therapy in patients with urosepsis causedby extended- spectrum beta-lactamase producingEscherichia coli and Klebsiella pneumoniae isolates.Material and Methods: Patients included in the study wereevaluated in two groups. Group 1 consisted of 14 patientswho underwent carbapenem therapy for 14-21 (median:14) days, while Group 2 consisted of 12 patients whoreceived carbapenem therapy for 3-11 (median: 6) days, followed by oral sequential therapy with ciprofloxacin ortrimethoprim-sulfamethoxazole. The clinical features andtreatment outcomes of patients in each group werecompared. Results: Patients in both groups were similar in terms ofdemographic and clinical findings. The causative agentwas E. coli in 9 cases while K. pneumoniae was isolated in17 cases. Clinical improvement was noted for 24 patientswhile relapse occurred in one case from each group. Conclusion: As an outcome we think that oral sequentialtherapy may be effective after carbapenem therapy inpatients with urosepsis caused by extended spectrum betalactamaseproducing bacteria and further evaluation withextended, prospective and randomized studies would behelpful.

Etken mikroorganizma, olguların dokuzunda E. coli iken, 17 olguda K. pneumoniae idi. Yirmi dört olguda iyileşme sağlandı. Her iki gruptan birer olguda relaps gelişti.Sonuç: Çalışma sonucunda, genişlemiş spektrumlu beta laktamaz üreten E. coli ve K. pneumoniae izoatlarının etken olduğu ürosepsis olgularının tedavisinde karbapenem tedavisi sonrası oral ardışık tedavinin etkin olabileceği ve konuyla ilgili daha geniş, prospektif ve randomize çalışmaların yararlı olacağı düşünüldü.
