Abstract

Background: It has been established that hemoglobinopathies and chemically modified derivatives of hemoglobin, interfered with some HbA1c assay methods either positively or negatively. In this study we present a case with a low HbA1c value as measured by ion-exchange HPLC method. Case presentation: A 55 year old patient admitted to policlinic of Internal Medicine and the result of HbA1c was 2.1%, measured by ion-exchange HPLC method in our laboratory. Unusual peaks and warning about variant hemoglobin were detected in the result chromatogram. Following on suspicion of hemoglobin variant sent to referral laboratory, was determined result of HbD-Los Angeles 84.4%, HbA 6.9%, HbA2 3.9%, HbF 4.8% (Trinity Biotech variant analyzer). Discussion: Variant hemoglobin may cause negative interference in methods such as ion-exchange HPLC, while immunological methods do not cause significant interference in the measurement. Therefore, in the presence of clinically incompatible HbA1c levels, hemoglobin variants should be investigated.
