Abstract

Objective: Chemotherapy has minimal activity in metastatic gastric cancer. Palliative chemotherapy is used but survival benefit is limited.Method: Ninthy-nine metastatic gastric cancer patients who had palliative chemotherapy between January 1990-January 2001 in Erciyes University Medical Faculty were evaluated retrospectively. Forty-one patients were treated with 5 fluorouracil (FU) 425 mg/m2 for 5 days andfolinic acid 20 mg/m2for 5 days every 4 week 6 cycles (FUFA), 14 patients FU 400 mg/m2 every week during 6 months (Weekly FU), 44 patients FU 600 mg/m2 days 1, 8, 29, 36, adriamycin 30 mg/m2 days 1, 29 and mitomycin 10 mg/m2 day 1 every 8 week, 6 cycles (FAM). Results: Seventy-one were men and 28 women. Median age was 59 (range 29-80) years. Sixty-two patients had adenocarcinoma, 35 had signet ring cell carcinoma and 2 mucinous adenocarcinoma. Metastatic sites were most frequently liver in 61 patients. Partial remission was obtained 17.2% of patients 21.2% of patients h ad stabile disease and 61.6% progressive disease. Treatment response was not statistically different between treatment arms. Median progression free survival (PFS) in all patients was calculated 6 ± 0.77 months (95% CI 4.49-7.51), and overall survival (OS) 8 ± 0.82 (95% CI 6.39-9.61). PFS and OS were shorter in Weekly FUgroups than PUPA and PAM groups but similar between FUFA and FAM groups. Hématologie toxicity and mucosi-tis were the most frequent side effect. Conclusion: Chemotherapy regimens have minimal activity in metastatic gastric cancer. We need to more effective chemotherapy regimens.

Özet

Amaç: Metastatik mide kanserinde kemoterapi düşük etkinliğe sahiptir. Palyatif kemoterapi uygulanmakta, fakat sağkalıma katkıları sınırlı olmaktadır. Yöntem: Erciyes Üniversitesi Tıp Fakültesi Hastanesi'nde Ocak 1990-Ocak 2001 tarihleri arasında palyatif kemoterapi uygulanan 99 metastatik mide kanseri olgusu retrospektif olarak değerlendirildi. Hastalardan 41 'i fluorourasil (FU) 425 mg/m2 vefolinik asit (FA) 20 mg/m2 1-5 gün, 4 haftada bir fluorourasil vefolinik asit kombinasyonu (PUFA); 14'ü, FU400 mg/m2 haftada bir 6 ay süreyle (HafFU) ve 44'ü FU600 mg/m2 l, 8, 29, 36. gün, adriamisin 30 mg/m2 l, 29. gün, mitomisin C 10 mg/m2 1. gün 8 haftada bir (FAM); ile tedavi edildi. Bulgular: Hastalann 71 'i erkek, 28'i kadındı. Median yaş 59 (29-80) yıl olarak saptandı. Hastaların 62'si adenokarsinom, 35'i taşlı yüzük hücreli karsinom ve 2'si musinöz adenokarsinomdu.
