Abstract

OBJECTIVE: Our aim was yo discus patients diagnosed with peritoneal tuberculosis, an antitiy easily confused with and after similar to extensive ovarian tumors, in the light of modern day literature. Design: We designed to discuss peritoneal tuberculosis alongside a case reportSetting: Gazi University Faculty of Medicine Obstetrics and Gynecology Department, Beşevler, AnkaraPatient: A female patient of 30 years applied to our clinic with abdominal distension and discomfort. Upon which a pelvic ultrasound was performed and pelvic ascite was discovered. Further tests didn’t reveal any pathological results. Diagnostic laparotomy showed miliary implants on all peritoneal surfaces. Cytological and frozen pathology testing confirmed the absence of a malignancy, leeding us to consider the possibility of peritoneal tuberculosis. Intervention: Our patients complained of intraabdominal ascite, distension and pain we decided to perform diagnostic laparotomy. Main outcome measures: Parameters of laparotomy ruled out malignancy and postoperatively the patients was diagnosed with peritoneal tuberculosis.RESULT: Peritoneal tuberculosis possess clinical manifestations that resemble those of extensive ovarian tumors. CONCLUSIONS: Peritoneal tuberculosis may present with non specific symptoms. As with the case discussed, abdominal pain, discomfort, presence of an adnexial mass, ascite and elevated CA 125 measure. Due to this extensive ovarian tumors must be considered as differential diagnosis.

Özet

Objektif: İleri evre over tümörü tanısı alan hastaların klinik bulgularını taklit eden peritoneal tüberkülozis olgusunun literatür eşliğinde tartışılması.Planlama: Peritoneal tüberkülozis olgusunun klinik bulgularının olgu sunumu eşliğinde tartışılmasıOrtam: Gazi Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim DalıHasta: Oyuz yaşında bayan hasta karın şişliği, ağrı şikayeti ile kliniğimize başvurdu. Kliniğimizde yapılan ultrasonografisinde pelviste yaygın asit saptanan hastanın ileri tetkiklerinde patolojiye rastlanmadı. Tanısal laparotomi uygulanan hastanın tüm peritoneal yüzeylerinde milier tarzda implantlara rastlandı. Sitoloji ve frozen incelemesinde malignite rastlanmayan hastada peritoneal tüberkülozis tanısı düşünüldü.Girişim: İntraabdominal asit, karın şişliği ve ağrı şikayeti olan hastaya kliniğimizde tanısal laparotomi uygulandı.Değerlendirme Parametreleri: Laparotomi sonrası malignite düşünülmeyen hastaya postoperatif dönemde peritoneal tüberkülozis tanısı kondu.SONUÇ: Peritoneal tüberkülozisin klinik bulguları ileri evre over tümörü tanısı alan hastaların klişnik bulgularını taklit edebilir.YORUM: Peritoneal tüberkülozis non spesifik semptomlarla klinik bulgular verebilir. Hastalarda karın ağrısı, rahatsızlık,adneksiyel kitlenin varlığı, asit ve yükselmiş CA 125 seviyeleri saptanabilir. Bu yüzden ayırıcı tanıda ileri ever over tümörü düşünülmelidir.
