Abstract

Objective: To investigate the diagnostic efficacy of office hysteroscopic observation in women presented with abnormal uterine bleeding because of endometrial hiperplasia and the correlation between pathology results and observation. Institution: Zekai Tahir Burak Women's Health Education and Research Hospital Material and method : Between January 2000 and march 2001, 216 premenopaıısal and 114 postmenopausal women with a mean age of 32,6 (+7) and 55(+7) presented with abnormal bleeding were accepted to the study. Focal or diffuse thickening of endometrium, irregular endometrial surface, cystic regions which are enlarged through uterine cavity and increase in vessel density on panoramic appearance used as a criteria for hyperplasia. Prediagnosis which were given using office hysteroscopy was compared with the pathology results. Results: In 50 (36 premenopaıısal and 14 postmenopausal) of the 70 (53 premenopaıısal, 17 postmenopausal) patients the lesions which were prediagnosed as endometrial hyperplasia using office hysteroscopy were verified by the pathology results. Using pathologic evaluation, 8 of 163 premenopaıısal and 4 of 97 postmenopausal patients were diagnosed as endometrial hyperplasia which were incorrectly prediagnosed using hysteroscopic observation as endometrium without hyperplasia. In premenopaıısal patients, false positive rate was 32% which was caused by proliferative endometrium( 11 patients) and secretuar endometrium 6patients In postmenopausal patients, endometrial atrophy and inflammation (3 patients) were found as the main causes of false positive results. In the diagnosis of endometrial hyperplasia office hysteroscopy had a positive predictive value of 70 %,and negative predictive value 98%. Discussion: If we assess the results entirely, the diagnostic competence of office hystemscopy is encouraging. Advanced diagnostic value, minimal trauma and short procedure time makes the office hysleroscopy an important diagnostic tool for the diagnosis and followup of endometrial hyperplasia.
