Abstract

Objective: The aim of this study was to compare office hysteroscopy, hysterosonography and hysterosalpingography (HSG)in terms of diagnostic accuracy, procedure time and patients tolerance while evaluating the uterine cavities of infertile women. Institution: Department of Infertility, Zekai Tahir Burak Women's Health Education and Research Hospital, Ankara Materials and Methods: This prospective randomized study included 63 patients admitted to our infertility clinic between January, 2002 and June, 2002. Male factor is ruled out using spermiogram. Mean age of 63 patients was 29, 7 (range 20-42). 39, 35%of the patients were secondary infertile and 60, 65% primary infertile. The mean duration of infertility is about 3, 9 year (range 1-11year Three outpatient uterine evaluation techniques were performed on 3 separate follicular cycle days of their current or next menstrual cycles. Operative hysteroscopy was performed in all patients who were found to have a uterine abnormality during any of the 3 procedures. Statistical analysis was used to compare diagnostic accuracy, subjective pain score (between 05) and procedure length between HSG, OHS and hvterosonography. Informed consent were taken from all patients. Patients were excluded if they had a suspected pregnancy, a history of iodine allergy or reactions and having active treatment for any sexually transmitted disease or pelvic inflammatory disease. Results:Mean age of 63 patients was 29(+ 0. 72 ). 40% of the patients were secondary infertile and 60% primary infertile The mean results of procedure times showed no significant difference. The average of procedure time was 5. 6 minutes for HSG, 8. 4 nün for hysterosonography and 6. 2min for office hysteroscopy. The main pain scores of HSG and office hysterescopy were 3. 11 and 3. 18, greater than that for hysterosonography which was 1. 57. This difference was statistically significant. In 36% of cases, pathological abnormality was found in the uterine cavity in one of these procedures. In 49% of the patients no abnormalities were, observed and in 24% of patients an abnormality was found using all three procedures. In 13% of the patients pathological abnormality was found in one of the procedures. In 13% of the patients an abnormality was observed using two of the three procedures. The results we/ ' confirmed by operative hysteroscopy and no statistically significant difference was found between these procedures in detecting the pathological abnormalities of the uterine cavity. Conclusion: While HSG was found to be less painful than the two other procedures, office hysteroscopy was the best in diagnosis of uterine cavity abnormalities. But none of these differences was statistically significant in evaluation of the uterine cavities of infertile women . In conclusion, HSG. office hysteroscopy and hysterosonography had the same value in this field.

Özet

Amaç: Infertil kadınlarda, uterin kavitenin değerlendirilmesinde ofis histeroskopisi, histerosalpingografi ve histerosonografinin tanısal doğruluk , uygulama süreleri ve hasta toleranslarının karşılaştırılması Çalışmanın Yapıldığı Merkez: Zekai Tahir Burak Kadın Sağlığı Araştırma ve Eğitim Hastanesi infertilité Kliniği Materyal ve ınetod: Bu prospektif randomize çalışmaya yaşları ortalaması 29, 7(aralık 20-42)olan infertilite şikayetiyle başvuran 63 hasta alındı. Erkek ınfertilitesi spermiogramla ekarte edildi. Hastaların %39,35'i sekonder infertil, %60, 65'i primer infertildi.
