Abstract

Objective: To evaluate the effect of body mass index on follicle selection and ovulation during spontaneous and induced cycles in infertile patients. Institution: SSK Ankara Maternity and Women 's Health Teaching Hospital, Materials and Methods: One hundred and eighty four consecutive infertile women were recruited to this prospective study and divided into five groups according to assigned treatment protocols. Thirty four patients were followed up without ovulation induction formed Group I, while 35,42,37 and 36 patients underwent a standard regimen of clomiphene citrate (Group //), human rnenopausal gonadotropin (Group III), pure FSH (Group IV) and long protocol with gonadotropin releasing hormone analogue (Group V), respectively. Cervical score, endometrial thickness, luteinizing hormone (LH), estradiol (E2) measurements and ovulation rates by midluteal progesterone levels were compared within the groups according to body mass index (BMI) classification as normal, overweight and obese. ANOVA, Student's ttest. Chi-square and Mann-Whitney U test were used as appropriate to compare mean values between groups. Results: In group I obese patients were found to have higher LH level (p=0.002), E2 level (p=0.005), cervical score (p=0.001) and endometrial thickness (p=0.002) when compared to normal and overweight patients. In groups II, III. IV, V to whom ovulation induction was performed, the LH level, the E2 level, the cervical score and the endometrial thickness measurements did not differ among normal, overweight and obese patients. The ovulation rates were not different in any group when compared according to BMI class. Conclusion In the presence of adequate gonadotropins, the clinical effect of BMI on follicle selection and ovulation is negligible in induced cycles.
