Abstract

Objective: To investigate the sex, age, follow-up time, and accompanying eye examination findings and the outcomes of vitreoretinal surgery in cases with dropped lens or lens fragment to vitreous cavity. Material and Method: Sixteen patients with dropped lens fragment or lens to vitreous cavity that were followed-up in Turkish Ministry of Health Ankara Ulucanlar Eye Education and Research Hospital Retina Clinic between January 2012 and June 2013 were retrospectively evaluated in this study. A complete ophthalmological examination including visual acuity, intraocular pressure with applanation tonometry, biomicroscobic examination findings, dilated pupil examination of the posterior segment, and ocular ultrasonography were evaluated a t baseline and during follow-up period. Results: The average age of patients was 69.1 years (46-88 years). There were 7 male (43.7%) and 9 male (56.2 %). Dropped nucleus during phacoemulsification in 10 cases (62.5%), post traumatic lens dislocation in 5 cases (31.2%), and spontan dislocation in 1 case (6.2%) were detected as etiologic causes. Vitreoretinal surgery including 23-gauge pars plana vitrectomy, clearing of lens or retained lens material, posterior hyaloid membrane removing, endolaser photocoagulation and fluid-air exchanged was performed. Intraocular lens implantation (IOL) was done in 10 cases (62.5%) during same surgery. Posterior chamber IOL implantation on to the anterior capsule in 5 cases (31. 2%), anterior chamber IOL implantation in 4 cases (25%), and scleral fixated posterior OL implantation in 1 case (25%) were performed. T he mean preoperative visual acuity was 2/100 (hand motions-0.2) and mean overall postoperative visual acuity was 0.3 (4/100-1.0). The m ean overall preoperative intraocular pressure was 14.6 mmHg (7-21 mmHg). The mean overall postoperative IOP was 11.7 mmHg (7-18 mmHg). The visual acuity recorded at the last follow-up was better than the first visit in 14 cases (87.5%), and the same in 2 eyes (12.5%). Through the follow-up visits, none of the cases developed endophthalmitis and retinal detachment. Conclusion: The worsened visual acuity in cases with dropped lens or lens fragment to vitreous cavity can be improved with vitreoretinal surgery combined intraocular lens implantation.

Özet

Amaç: Vitreus içine lens veya lens parçası düşen ve vitreoretinal cerrahi (VRC) uygulanan olgularda yaş, cinsiyet, eşlik eden göz mua yene bulguları ve uygulanan cerrahinin sonuçlarını incelemek. Gereç ve Yöntem: Ocak 2012-Haziran 2013 tarihleri arasında Sağlık Bakanlığı Ankara Ulucanlar Göz Eğitim ve Araştırma Hastanesi’nde vitreus içine lens düşmesi tanısı alarak VRC yapılan 16 hastanın 16 gözü incelendi. Ameliyat öncesi ve sonrası görme düzeyleri, göz içi basıncı değerleri, biyomikroskopik bulguları, pupil dilatasyonu sonrası göz dibi muayenesi ve oküler ultrasonografiyi içeren tüm göz muayene bulguları değerlendirildi. Bulgular: Çalışmaya dahil edilen olguların ortalama yaşı 69,1 yıldı (46-88 yıl). Olguların 7’si erkek (%43,7), 9’u kadın (%56,2) idi. Etyolojilerine göre değerlendirildiğinde; 10 olguda (%62,5) fakoemülsifikasyon cerrahisi sırasında, 5 olguda (%31,2) travma son rası ve 1 olguda (%6,2) spontan lens düşmesi mevcuttu. Bütün olgulara 23 gauge triamsinolon eşliğinde pars plana vitrektomi, lensin veya kalıntılarının temizlenmesi, arka hyaloid ayrılması, 360 derece endolazer fotokoagülasyon ve serum hava değişimi uygulandı. 10 olguya (% 62,5) a ynı seansta göz içi lens (GİL) implantasyonu yapıldı.
