Abstract
Gunshot injuries are rare causes of traumatic arteriovenous fistulas and the main treatment method is surgery. A forty two years‐old male patient presented with chest pain in the chest area near the right shoulder. In the examination of the patient, 6/6 continuous murmur and thrill were detected in the right subclavian region. Selective angiography performed for the right upper extremity revealed 2 fistulas between the subclavian artery and vein. The fistula was successfully closed percutaneously with a graft stent. According to the literature research; this case is the first example of successful repair of arteriovenous fistulas caused by gunshot injury by percutaneous intervention.
Özet
Travmatik arteryovenöz fistüllerin nadir nedeni ateşli silah yaralanmasıdır ve temel tedavi yöntemi cerrahidir. Kırk iki yaşında erkek hasta sağ omuz başına yakın göğüs bölgesinde oluşan göğüs ağrısı şikayetiyle kliniğimize başvurdu. Hastanın yapılan muayenesinde sağ subklavyen bölgede 6/6 devamlı üfürüm ve thrill tespit edildi. Sağ üst ekstremite için yapılan selektif anjiyografide subklavyen arter ve ven arasında 2 adet fistül tespit edildi. Fistül perkütan yolla greft stent aracılığıyla başarılı bir şekilde kapatıldı. Yapılan literatür araştırmasına göre bu olgu ateşli silah yaralanmasına nedeniyle oluşan arteryovenöz fistüllerinin perkütan girişim ile başarılı onarımının ilk örneğidir.
