Abstract

We report a case of isolated tuberculous epididymoorchitis developingin a patient after haematopoietic stem cell transplantation (HSCT).Forty-four-year-old male was admitted to the hospital with scrotalpain and swelling 6 months after an allogeneic HSCT using a fullymatchedsibling donor because of his acute myeloid leukemia. Therewere scrotal tenderness, thickening and erythema on the right side.Brucella standard tube agglutination test was negative. Increasedscrotal skin thickening, edema in the right epididymis and increasedtesticular vascularization were detected on ultrasonography. He wasreadmitted to our hospital with recurrent scrotal pain after 3 monthsof partial improvement with oral ciprofloxacin administered for adiagnosis of right epididymoorchitis. Pelvic magnetic resonance imagingrevealed bilateral epididymoorchitis and scrotal abscess. Acidfast bacilli were detected on Ehrlich-Ziehl-Neelsen staining of thecontent of abscesses drained under local anesthesia. Mycobacteriumtuberculosis complex was isolated on the 24th day of quadruple antituberculosistherapy. Scrotal fistula developed on the first month oftherapy which healed spontaneously after discontinuation of immunosuppressiveagents. Full recovery was achieved after six months ofantituberculosis therapy. As a result, tuberculous epididymoorchitisshould be kept in mind in the presence of chronic epididymoorchitisdeveloping in patients receiving immunosuppressive therapy.

Özet

Bu yazıda alojenik hemapoetik kök hücre nakli yapılan hastada gelişen izole tüberküloz epididimoorşiti sunulmuştur. Kırk dört yaşındaki erkek, akut myeloid lösemisi nedeniyle tam uyumlu erkek kardeşinden alojenik kök hücre nakli yapıldıktan 6 ay sonra skrotal ağrı ve şişlikle başvurdu. Sağ skrotal bölgede ciltte hassasiyet, kalınlaşma ve eritem vardı. Brucella standard tüp aglütinasyon testi negatifti. Ultrasonografide skrotum cilt kalınlığında artış, sağ epididimde ödem ve testis kanlanmasında artış vardı. Sağ epididimorşit tanısıyla başlanan oral siprofloksasinle kısmen düzelen hasta 3 ay sonra yeniden skrotal ağrıyla başvurdu. Pelvik magnetik rezonans gö- rüntülemede bilateral epididimoorşit ve bilateral skrotal apse saptandı. Lokal anestezi altında drene edilen apse içeriğinden yapılan Ehrlich-Ziehl-Neelsen boyamasında aside dirençli basil görüldü. Başlanan dörtlü antitüberküloz tedavinin 24. gününde Mycobacterium tuberculosis kompleksi izole edildi. Tedavinin birinci ayında gelişen skrotal fistül, kullanmakta olduğu immünosüpresif tedavilerin kesilmesi sonrasında spontan olarak kapandı. Altı aylık antitüberküloz tedaviyle tam iyileşme sağlandı. Sonuç olarak, immünosüpresif tedavi alan hastalarda gelişen kronik epididimorşit karşısında tüberküloz epididimoorşiti de akılda tutulmalıdır.
