Objective & Hypothesis:Objective: This study compared the preoperative findings of high resolution temporal bone computed tomography (HRCT) withotoscopic-microscopic examination in chronic suppurative otitis media (CSOM) cases which are not complicated with intracranial orextracranial complications.Patients and Method: Adult 82 CSOM cases, without any history of otologic surgery, operated on canal wall up (CWU) or canal walldown (CWD) tympanomastoidectomy were analysed retrospectively.

Abstract

Objective: This study compared the preoperative findings of high resolution temporal bone computed tomography (HRCT) withotoscopic-microscopic examination in chronic suppurative otitis media (CSOM) cases which are not complicated with intracranial orextracranial complications.Patients and Method: Adult 82 CSOM cases, without any history of otologic surgery, operated on canal wall up (CWU) or canal walldown (CWD) tympanomastoidectomy were analysed retrospectively. Preoperative HRCT findings were compared with intraoperativefindings. The matching of findings (true positive, false positive, true negative, false negative, sensitivity, specificity, positive predictive valueand negative predictive value) was analysed to understand which of the findings is most compatible. Results: HRCT was found to have high sensitivity for diagnosis of cholesteatoma (85.2%), ossicular chain discontinuity (80.4%), lateralsemisircular canal (LSSC) dehiscence (100%), scutum erosion (85.2%), tegmen tympani erosion and posterior fossa dural plate erosion(100%), however it has a low sensitivity for diagnosis of facial nerve canal dehiscence (52.9%). The specificities of these lesions with HRCTwere calculated as 100%, 100%, 97.5%, 100%, 96.1% and 100%; respectively. Conclusion: HRCT alone was found to be insufficent in diagnosis of facial nerve canal dehiscence. However, HRCT is still anindispensable imaging modality to plan for accurate surgical technique -including additional procedures as ossicular chain reconstruction andbony defect repair- and to minimize intraoperative complications.

Özet

Amaç: Çalışmamızda; intrakranial veya ekstrakranial komplikasyonların eşlik etmediği Kronik Süpüratif Otitis Media (KSOM)'lı olguların, otoskopik-mikroskopik inceleme birlikteliğinde Yüksek Çözünürlüklü Temporal Kemik Bilgisayarlı Tomografi (YÇBT) iledeğerlendirilmesinin tedavi sürecine etkisi araştırıldı.Hastalar ve Yöntem: KSOM tanısı ile CWU (Canal Wall Up) veya CWD (Canal Wall Down) mastoidektomi teknikleri ile opereedilmiş, daha önce cerrahi girişim uygulanmamış, erişkin yaş grubunda 82 olgu retrospektif yöntemle gözden geçirildi. Preoperatif YÇBTbulguları ile operasyon sırasında tespit edilen bulgular karşılaştırıldı. Bulguların uyumluluğu incelenerek, [Gerçek Pozitif, Yalancı Pozitif,Gerçek Negatif, Yalancı Negatif, Duyarlılık (Sensitivite), Özgüllük (Spesifite), Pozitif Prediktif Değer-PPD, Negatif Prediktif Değer-NPD]özellikle hangi bulguların önem arz edeceği değerlendirildi.Bulgular: YÇBT'nin kolesteatoma varlığı (%85.2), kemikcik zincir defekti (%80.4), lateral semisirküler kanal (LSSK) destrüksiyonu(%100), skutum erozyonu (%85.2), orta veya posterior kafa çukuru kemik duvar defekti (%100)'nin tanımlanmasında yüksek, fasiyal sinirkanal dehissansı (%52.9)'nın tanımlanmasında ise düşük bir duyarlılığa (sensitivite) sahip olduğu bulundu. Özgüllük (spesifite) oranları isesırası ile %100, %100, %97.5, %100, %96.1 ve %100 hesaplandı.Sonuç: Fasiyal sinir kemik duvardaki dehissansların tanımlanmasında YÇBT'nin tek başına yeterli olmadığı görüldü. Planlanacakcerrahi tekniğin seçimi, kemikcik zincir rekonstrüksiyonu, kemik duvarlardaki defektlerin onarımı gibi uygulanacak ilave işlemlerinöngörülebilmesi, karşılaşılabilecek intraoperatif komplikasyonların en aza indirilmesi koşulları birlikte ele alındığında, YÇBT'ninvazgeçilemez bir değerlendirme yöntemi olduğu sonucuna varıldı.
