Abstract

Malacoplakia is rare type of inflammatory reaction with an unknown etiology. Although it can be seen at any genitourinary and nongenitourinary location, bladder is the most frequent site of involvement. It is usually characterized as multinodular thickening of mucosa and submucosa of the trigon areas and may be confused with carcinoma. Microscopically there is mixed inflammatory infiltration that predominantly consists of subepithelial histiocytes with granular acidophilic cytoplasm. Some of these histiocytes contain intracytoplasmic inclusions, which can stain with PAS, iron and calcium stains. Long-term follow-ups revealed that these characteristic old lesions turn into fibrosis and scar tissue. Forty-nine years old male patient with dysuria and microscopic hematuria was assessed for a tumor-like lesion of 7 cm in diameter at anterior wall of the bladder. This patient has undergone double TUR, TRU-CUT and frozen section to approve malignancy. However, all these procedures resulted with chronic nonspecific cystitis without any evidence of malignancy. Under these circumstances patient had partial cystectomy for the lesion. This rare case was presented with the review of the recent literature.
