Abstract

Objective: To evaluate the effects of polyhydramnios on maternal and fetal outcome in preeclamptic pregnancies Materials and Methods: A total of 435 preeclamptic women, 12 with polyhydramnios and423 controls were included. Groups were compared regarding the occurrence of HELLP syndrome, eclampsia, other maternal complications, requirement for antihypertensive or magnesium sulfate therapy, acute fetal distress, perinatal mortality, and neonatal morbidity including admittance to intensive care unit, hypoglycemia, hypocalcemia; sepsis, convulsion, and intracranial hemorrhage. Mann-Whitney U test, chi-square tests, and logistic regression analyses were used for statistical evaluation. Results: Polyhydramnios was present in 2,8% of the preeclamptic cases. Maternal blood hematocrit and calcium values were significantly lower in women with polyhydramnios (p=0.02 and p=0.03, respectively). Neonatal hypoglycemia and hypocalcemia were more frequent with polyhydramnios (p=0.006 and p=0.04, respectively). None of the patients with polyhydramnios had developed eclampsia (not significant). Logistic regression analysis yielded no association between polyhydramnios and maternal-fetal morbidity. Conclusions: Polyhydramnios seems to be unrelated with maternal-fetal morbidity and mortality, in preeclampsia. Polyhydramnios may be protective against the development of HELLP syndrome and eclampsia in women with preeclampsia.
